A Poor Substitute

As the details about the disastrous Maui fire continue to unfold, many people are left wondering how the hell a foul-up this spectacular is possible. How did everything go so wrong at every level of the response? Why did Maui have to wait for water authorization? Why were the emergency sirens not activated? Why was traffic allegedly blocked from leaving on the only good road out? Assuming that all of what we've heard is true, how did so much go wrong?

I think that the sad conclusion is that, in reality, nothing went wrong. Not officially. Everyone likely followed every guideline, rule, regulation and requirement. Procedural compliance was probably quite good. What apparently nobody did, however, was the single most important thing: think. The environment in which they operate has been designed to eliminate the need to think and ensure compliance instead.

The purpose of the rules laid down in such great volume for so long a time is to obviate thinking. You don't need to think if there's a rule to cover whatever it is you need to do. Follow the rules, comply with the procedures, adhere to the regulations and you will achieve your goal or at least be shielded from any poor consequences. Nobody is to blame, because the rules were followed. The rules are there to act as a substitute for thinking. Robotic adherence to the rules - and striving to have a rule for all things - is awfully bad in situations that even slightly deviate from the context envisioned by the rulemakers.

The water situation in the Maui fire response is, I think, an unfortunately spot-on example. Why did the local authority on Maui call Honolulu for permission? Because the rules said it needed to. Why did Honolulu allegedly keep Maui waiting for five hours? They were busy, I'm sure. And the rules permitted it. Why didn't the local incident commander or similar grab the water guy by the lapels and tell him to get the damned water flowing or else, and pick up the pieces later? That would be against the rules. Why didn't they use the emergency sirens? No rule for that, not for a fire. The rules are for tsunami warnings. No rule, no action. Everyone apparently followed the rules. If the rumors that kids were sent home from school into the path of the blaze because of high winds or that firefighters left the scene before the fire was fully quelled are true, there were probably rules for that, too.

Maui isn't the only example. The Flint, MI municipal water disaster is another. Flint wanted to change the water source to reduce costs. Both water sources were safe. The chemistry was a bit different between the two sources, but both were fine and complied with safety and quality standards. All of the regulations were satisfied. All the rules were met. But nobody thought about it. They were ticking boxes, not thinking about whether the difference in acidity would cause built-up scale in the pipes to release and expose the water supply to lead. There was no rule for that, and disaster followed.

The world is a complicated place and it is impossible to have a rule - or set of rules, or set of sets of rules ad infinitum - that will cover all conditions. But bureaucracy runs on rules and metes out reprisals for breaking them - even if breaking a rule or deciding not to proceed despite the rules saying you can would be the single best thing to do. Rule-following is in the organizational DNA at all levels, and it has made thought redundant.

And box-ticking is a poor substitute for thinking.

We now have political leaders and bureaucrats who are unwilling and likely incapable of acting fast, reacting to emergencies with speed and creativity. The gal responsible for the parts was a little miffed she wasn't consulted, but said, "I'm all for the rules, but I'm okay bending them if needed. Just let me know first".

1st, what sirens were used during that incoming attack snafu a few years back?

2nd, I've read they have one tsunami every 30+ years, so it's short-sighted to have a system in place and almost never use it. I would bet if they had used the sirens it would have alerted people to check internet/tv/radio where they would quickly learn it was about fires.

Article in Morning Report said they were for a variety of things, including fire. So if they worry was, "Well, they'll think it's a tsunami warning an run INTO the fires!", well, the alarms were for more than just Tsunamis and air raids.

I don't think it's even that thought out. A bunch of rules to produce a certain result consistently is a process. Most rules for organizations are simply accumulations over time(put in place reactively or for some purpose not always relevant), not core processes in producing a product.

Lack of a decisionmaker/leadership (or perhaps lack of an effective one) with authority to make calls on the fly seems to be what was absent here.

Which can still be potentially useful. Though the expense is substantial and there seems to be limited use-cases where it does things we couldn't do before.

Maybe a software-development aide for first drafts that are then optimized.

There was not a leader to be found. That's the point of most organizations and the point of virtually every layer of government. They don't want leaders. They don't need them. They have rules. Just follow the runbook.

I learned this when I failed every time I tried to work for an electronics retailer (Best Buy, Circuit City, CompUSA - all of them). I failed out during interviews every time. I asked I guy I met who was a Best Buy manager what I was doing wrong. He told me, "never, ever say you might be interested in management at some point. The "book" said that all devices having maintenance had to be "tagged out". Basically, the breaker had to be shut off before work could be done. This involved getting the log book, filling out the paperwork, signing the tag, getting a witness to also sign, and placing the tag on the circuit breaker.

However, in one particular instance, there was a piece of gear that for maintenance you had to test if it operated correctly by turning the power on. So, you had to go through all of the steps to turn OFF the power, only to have step one of the actual procedure be to turn ON the power.

I doubt it, and the reason I doubt it is that an operation at that scale with that level of compliance would be hard to achieve, particularly on short notice. The right conditions for a fire, the right people in the right positions to ensure a perfectly-flubbed response, someone to set the fire, nobody having an attack of basic humanity and deviating from the plan, no leaks about coordination, etc.

It strikes me as unlikely in the extreme.

In one of the Data Centers I worked at the procedure for a fire in the machine rooms was roughly 20 pages long and was supposed to be followed chapter and verse from start to end. iirc, the Halon system would kick off after 8 seconds or so of the alarm. Indian cuisine is much more varied than Americans think. There are hundreds if not thousands of regions in India and each has its own styles and cuisine. Some you would never think of as "Indian" food, just like some of the food in Florence you'd never figure for Italian. The "Indian" food most Americans think of is the equivalent of Chinese takeout: created for a mass-market American audience.

There are a few "real" Indian restaurants, but they are the exception rather than the rule. How can you identify them? Look at who is eating there. If the clientele is mostly Indians, then it's a good bet the place is legit. If not, then the place is McPatel's.

We had Oxygen Breathing Apparatus. It didn't have a tank of oxygen, but rather a large metal cartridge that used a chemical reaction to create oxygen. You wore it on your chest, and you had to insert the cartridge and use a lever to seat it, which broke a thin metal seal on the top. The moisture in your breath activated the chemical reaction.

They only lasted for so long, and to had to swap out the cartridge. You had to be careful when ejecting the old one, because it'd be super hot from the reaction.

By the by, I've never had food poisoning in an Indian restaurant and the restaurants that I've had the most food poisoning in have been KFCs(!).

I think that some KFCs try to push that leftover chicken just one more day by refrying the day old chicken again.

Functionally those who would be authorities themselves are not particularly bound by it but exercising their own judgement.

There is no presumption of favor to the replacing or the replaced government. In that context, between competing authorities, the rank and file Christian must exercise their own judgement.

One who is capable of being an authority must make their own judgement as to whether the cost of revolution to the people is worth the benefits. It's what they're more naturally capable of to begin with.

People with different resources and capabilities have different obligations and guidelines to follow. "If anyone causes one of these little ones—those who believe in me—to stumble, it would be better for them to have a large millstone hung around their neck and to be drowned in the depths of the sea" applies to rulers and the masses as much as it applies to regular children.

Well I inserted "fair and just" for ordained by God, and that resistance yielded damnation. I think that is the language in question. To say ... "but at times resistance is OK and even preferred" is inserting "text" that does not exist.

I forget if Paul was in prison when he wrote Romans ... certainly he was warned by prophets "don't go to Jerusalem" but he went anyway. ... But Romans 13 sounds like a hostage transcript that found its way into the text. heh

Also, the individuals weren't acting as individuals, they acted within the realm of casting off a corrupt government and replacing it with a Just government.

Joshua was sent into Canaan to wipe out all of the competing governments, not because of Joshua's personal ambition and rebellion to a particular government, rather as a practical matter of what do you do with millions of people who are antithetical to the existing establishment.

That is incorrect. God creates the individual in His image granting the Rights we recognize. He brings the individual to faith - the Church is just a way of describing the totality of those actions. Community is similarly just a way of describing a bunch of individuals.

Family is only of value to the extent that it doesn't contradict the individual's obligations to God. Jesus was quite clear about the fact that His Gospel would break up existing families as conversions happened.

Such good comments here, and questions, that probably no one will answer. The reason some rules were followed and some weren't is clear to me: the rules that punished the people were enforced, the rules that protected the people (like clearing flammable grasses) were not. To them, the people are the enemy. Never been clearer.

346 While I agree with you regarding rules, the problem is how many stupid people there are. Often, rules get invented b/c someone did something so stupid in a situation, a rule had to be created to ensure that didn't happen again. So, it's a circular type argument often - we wish there were more people who could think for themselves and not so many rules, but we run into idiots who without rules, will always make the wrong decision.
