One of the new weight-loss drugs, Wegovy, reduces heart attacks and strokes "in people at high risk."

I mean, to some extent, that's obvious: If you lose weight due to whatever reason, you're going to improve your cardiovascular health.

But these findings may make these drugs coverable by insurance and more readily available.


Wegovy, a weight-loss drug from Novo Nordisk, also helps prevent heart attacks and stroke in people at high risk, according to a company news release.

The trial of more than 17,000 adults 45 and older with obesity or overweight showed a 20% reduction in first heart attack, stroke or death from a cardiovascular event. Participants were given weekly injections of 2.4 mg of the drug semaglutide, sold as Wegovy, or a placebo, for as long as five years.

The results, released Tuesday, have not yet been peer reviewed or published in a scientific journal, and the Danish pharmaceutical company only provided limited information.

Doctors who treat patients with obesity were thrilled by the report, which is the first to show a major health benefit for a class of drugs first approved to treat people with diabetes.

"Obesity treatment is far more than about weight loss. It's about health improvement. It's about extending life," said Dr. Michael Albert, an obesity specialist in Oklahoma who runs a telehealth medical practice called Accomplish Health. "I think we'll look back and see this was a watershed moment (in the treatment of obesity)."

Albert, who has consulted for Novo Nordisk in the past, said he thinks the new trial will be enough to convince the federal government to cover the cost of Wegovy and weight-loss medications, known as GLP-1 agonists, which can run $1,300 a month or more without insurance.

Novo Nordisk's stock rose nearly 16% on Tuesday on the news of the study's result.

This plagues me: 80% of the time after I fall asleep, I wake up 10-20 minutes later.

Something that 70% of us experience, yet very few of us have even heard of: "hypnic jerks." Also known as sleep starts, sleep twitches or myoclonic jerks, the phenomenon tends to occur in the 10 minutes or so between the moment you go to sleep and the moment you actually fall asleep. As you're lying there with your eyes heavy and your breathing nice and rhythmic, your muscles suddenly contract in violent fashion, ripping you from your rest.

Sound familiar? It might not because a third of adults don't experience hypnic jerks, and many who do simply roll over and into their official sleep cycle (a little confused, but none the worse for wear). Still, others are familiar with the full suite; hypnic jerks can include hallucinogenic elements like perceived bangs or stomps and sensations of falling off a cliff or getting hit by a truck. I've had the former before -- I wouldn't recommend it. Some sleepers may even cry out when this happens. If you share a bed with a partner, you may have witnessed it firsthand.


It's possible that this all sounds unpleasant, if not a little alarming. In an age of careful and considered sleep hygiene -- with sleepers spending more time and money to make their little sleep havens as dark, cool and tech-less as possible -- should we be making a point to stamp out hypnic jerks, too? Are they negatively impacting our sleep patterns? The answer is more or less no. Sleep experts stress that research into hypnic jerks is light, but that's expressly because it's a harmless occurrence.

It's not harmless when it takes you 30 minutes to fall back asleep.


If hypnic jerks become a more frequent or intense in your wind-down routine, consider limiting your afternoon caffeine consumption. Other triggers: nicotine, exercise too late in the day...


As Bond told M in Never Say Never Again, approximately: "Very well, sir, I shall cut out the exercise."

No but I think I'll stop vaping an hour before bed, see if that improves things.

...stress and anxiety (e.g. working before bedtime, or laying there litigating the entirety of your day). Fortunately, the everyday switches that decrease your chance of a severe hypnic jerk will help you sleep better, anyway.

Maybe I'll get this book to help me sleep.

I saw something earlier this week, a headline that said something negative about, I don't recall what. My first thought was though, they don't want people taking this. It's too effective, and it helps people get off worse/more debilitating drugs, that are associated with obesity.

Because that's how I view the Pharmaceutical Industrial Complex these days, an organized crime syndicate that keeps people dependent and docile.

Had them for a long time before last doc told me it wasn't migraine and gave me the correct diagnosis. Oddly, the treatment is more caffeine--so I have a couple of cups of strong tea or take a caffeine tablet before bed. So grateful for the dx, because the caffeine works like a charm.

There's a guy I really like on YouTube called Mike Israetel. He's a PhD in Sport Physiology. He does a lot of no-BS on all sorts of health and fitness topics. He did a really good one on "Healthy at any size" that introduced a lot of nuance.

For example, he points out that you can be young, fat, and have good lab results indicating you're healthy ... for a few years. But then it catches up with you in a major way. Those same people five years later have tons of issues if they don't fix it quickly.

He also points out that you don't have to look like a marathon runner to be healthy and fit. If you reduce your fat, increase your muscle, and eat decent food, you can still be a bit overweight and have good lab results indicating you're healthy.

I'm within four pounds of "Normal Weight" for my height ... so that is nice. Need to lose another 15 to be under 200 for the first time in ? a couple decades or so. Also getting 10k steps in, thanks to my Fitbit watch keeping score. ... it's all good.

I've been working on that the last few weeks. Making myself get up 30 minutes earlier to do some stretches, sit-ups, etc., then doing some walking in the evening. It isn't much but it's better than nothing.

There is an old diagnosis that's gone out of disfavor, but was popular in the last century called "Masked Depression."

It basically means, you're not exhibiting any symptoms we associate with depression, but the doc thinks you are.

To say that anti-depressants are over-prescribed is one of those massive understatements that becomes silly to even talk about.

Eh, who knows? Deuterium is in normal water. DDW claims it has less Deuterium than normal water. I think if you're not already eating ridiculously clean in every other way, you probably have lower fruit to pick.

I spent about five minutes looking into it. DD water website sez D2O molecules are 'bigger' than H2O molecules, bad for mitochondria. I went about looking up the atomic radii and all that shit, and quickly came to my senses and shelved the issue. Bongino takes every supplement he can find.

124 Got my new bionic (aka artificial) knee a couple months ago. My gainzzzz are rehab progress to the point where I’m now back to jogging short distances… jogged 2 miles last evening. I say jog rather than run because I’m intentionally easing in and my pace is very slow…. Like around 11 minutes per mile, maybe a few seconds faster. Plus I mix in walking, so I covered 6 miles with 2 miles of jogging sprinkled in; knee feels solid.

I love finishing an intense workout around 7 pm then falling asleep around 10 pm… it helps my sleep I think. Maybe that’s not “too close” to bedtime

Also still plenty of cycling. I’m planning a 60 mile ride tomorrow

167 I typically have no problem falling asleep. I also use an APAP machine (it's an automated CPAP... it can change the PSI.) What happens is that routinely at 3-4AM I wake up and cannot get back to sleep.

I figure it's due to alcohol and nicotine. Neither of which I'm presently willing to go without. But will attempt to moderate in moderation.

As for the "hypnic jerks", I have a repetitive dream... probably several times a year where I'm driving a car on a winding road with traffic, going fast, very steep hills and as I crest one, I go flying off into the unknown. Pretty trippy. I actually like those. It's like being on a death rollercoaster. But without the death. Jerks me right awake every time.

I've learned the hard way this week that making a good stupid, silly comedy is very hard indeed. If not pure luck.

Example 1: "Young Einstein" is an aggressively stupid comedy but one of my favorites. It's very intelligently written and funny.

I watched Yahoo Serious' follow-up "Reckless Kelly' and was bored out of my mind. Very stupid, not funny, highly leftarded.

Example 2: "Pee Wee's Big Adventure" is aggressively silly but funny with lots of intelligent stupidity.
"Pee Wee's Big Vacation" made by Netflix isn't as bad as Big Top Pee Wee" but still not very good and kind of boring.

Not only is comedy hard, but stupid comedy is nearly impossible it seems.

216 Went out drinking last night (had a couple of NA beers) and then when I got home at 11:30 p.m., decided I'd convection vape some ganja. Was up until 1:15 a.m. playing guitar and eating some protein. Went to bed. Got up late (8:15) and decidedly did not feel like doing leg day. So I pedaled for an hour ten instead, worked up a good sweat.

You don't need to beat yourself up working out, but you do need to be consistently doing something. Weights tomorrow.

I can't endorse it. It was an attempt by Ritchie to get back to his 'funny gangster' roots but I hated most of the characters, especially the protagonist.

There's also a gay subplot with two of the side characters that felt forced. Not like...surprise anal forced but...close.

Okay. Lock Stock was 1998. Snatch was 2000. Now look up when the two Holmes movies, which he didn't write, came out. Personally, I don't like the Holmes movies, but even if you do, there's still years of mediocrity in between. The Gentleman, which is okay (and probably the best he's done since Lock Stock and Snatch), was a few years ago. Guy Ritche is uneven to say the least.

Four distinct seasons in PA is not all bad. None of them last too long.

Also, certain movements are hard to get right. People can squat for years and never do it well.

That'll do it. Maybe also just a general less moving around. All those little walks here and there and sitting standing and cleaning and mowing etc etc can be the diff btwn slightly abive and slightly below maintenance. And over time that can add up. Quick.

Also, I think eating time is a big deal. I have zero evidence other than personal experience and the experience of others. Also low quality sleep.

Same here. Don't know if the VA would prescribe me Wegovy but they were down with giving me a shit load of anti-depressants that initially worked but then they had to keep changing types/doses up - so sick of it. I know a lot of hordlings won't agree with this but I started with edible gummies. One gummy has 10mg of THC and I take half of one at night. Sleep like a dream and wake up feeling great - I too had hypnic jerks like I was falling. It would scare me to the point where I was only getting maybe 4 hours of deep sleep a night. I Threw away all of my anti-depressants. Started several months ago with resistance bands (I absolutely have always hated gyms) and power walking. Haven't lost weight (boo!) but starting to feel more toned.

This is so interesting. I have this happen a lot. I alwasy thought it was like synapses flashing. Sometimes I get teh falling sensation which makes me jerk. It happens so often it doesn;t really keep me from falling asleep. I tried to describe it to my doctor once and he started to freak that I was having mini strokes that I never mentioned it again.

I could probably squeeze out 35 if I had to. My PU routine is regular/narrow/wide x2 Usually it's 25/15/30 depending on other factors. I hate pushups, but I do them. Long gone are the days when I did 100 in 2 minutes.

320 I get the jerk awake just as I fall asleep occasionally. It usually happens either with a short dream or I wake up swearing I heard a sharp sound (like knocking on a door or window). There’s a name for that phantom sound, but I don’t recall it. As long as I don’t get an adrenaline pulse from a bad dream, I usually fall right back asleep.

I actually get this every once and a while. I never knew the two were the same thing though. I figured the noises/falling thing was just part of being in that hazy near dream stage, while the random leg twitch was something else.

Im also with you on approving of the pig thing. Some guys use foam blocks, so guessing this guy just did this as a goof. If it makes noise it actually isn't a terrible idea, as it'll warn you to stop lowering the weight, while not cheating by having a dense object to help you stop the weight, so you're doing the work and not having a weak point.

I'm not a doctor, but I am a layman who is wise to the deceptions of the Medical-Indusrial complex. I'm going to let you in on a dirty little secret. The Wegovy claim is quite typical. It's probably even true -- according to the study. But as the old Tom Waits song says, the big print giveth and the small print taketh away. Rarely asked are important questions like "What does it do to the overall death rate?" And in many cases, the answer is a drug touted for preventive measures scarcely changes it, or perhaps even raises it. Sometimes that rather important endpoint is not even mentioned in a study! Note that a drug might potentially still be useful even if it had high risks. But something pitched as prevention? No thanks. This was especially the case with statins, also marketed to reduce cardiovascular problems. And they do, but only to a small extent. And the overall risk of mortality? Almost no change.
