The Moral Abdication of The Academy [Muldoon]

A few days ago I ran across a brief news item out of Aspen, Colorado. A pair of campers on a rugged back country hiking trip along Conundrum Creek Trail in the Maroon Bells area ran into a bit of difficulty with some late spring snowfall and cold temperatures. Some of their camping equipment broke, they could not prepare food, and thought they might be getting frostbite. So they did what any sensible 25-year-old would do, they used their cell phone and called for help. Pitkin County Sheriff's office deployed deputies and a mountain rescue team of some 17 individuals to locate the two campers on the trail about six miles in, and escort them (uninjured and able to walk on their own) back to civilization. I guess there is a reason it is called Conundrum Creek.

I tell you that story so that I can tell you this story:

What image comes to mind when you think of a Pediatrician? Perhaps it's the jovial balding man with the plaid jacket and the bow tie who dons a clown nose and tells 'dad jokes' in the exam room. Or the earnestly pleasant fellow who insists, "Call me Doctor Tim!", and makes duck-quacking sounds to amuse the children. Maybe it's the matronly "Aunt Mabel" type who gently admonishes you on the best way to treat diaper rash. Or the fresh-faced young woman with the child-sized stethoscope draped around her neck who coos and babbles at your newborn? Authoritative or jovial, stern or easygoing, pediatricians come in all shapes and sizes, but surely must be benign and lovable, because they take care of babies, right? Everybody loves a baby goat or a baby elephant, so who wouldn't love a baby doctor?

The American Academy of Pediatrics (AAP), founded in 1930, is by far the largest professional association for Pediatricians in the USA, proudly describing their membership thusly:

We are 67,000 pediatricians committed to the optimal physical, mental, and social health and well-being for all infants, children, adolescents, and young adults.

While certainly the AAP and its various sub-committees have been instrumental in many advances in the medical and surgical treatments of major medical problems affecting children, the organization has a less savory side.

When I began my pediatric training in the mid 1980s it was expected as a matter of course that pediatric residents would automatically join the AAP. It was axiomatic that "pediatrician = AAP member". The mantra in those days was that we were to be "advocates for children". The pediatrician was to utilize his expertise to enhance, fortify and augment parents' ability to raise healthy children. The pediatrician and the parents, as a team, endeavored to help children become functioning adults, with well-rounded physical, mental and social capabilities. We were taught to respect the cultural and ethnic background of each family. Somewhere along the way, there began a shift from augmenting and enhancing, to overruling and usurping parents.

The AAP issues numerous policy statements, intended to address particular pediatric health issues authoritatively, from a position of being subject matter experts. These policy statements originally served as 'guidelines' for pediatricians, but have evolved to become the 'standard of care' for general pediatricians in their daily practices. AAP positions are cited as gospel and deviations from them are shunned. Busy pediatricians nearly always defer to the expert position, as the path of least resistance. Also, AAP statements are not just aimed at practicing pediatricians. More and more they are aimed at public policy makers, legislators and government agencies. I have been noticing some unsettling trends and techniques among the AAP's statements. Let's look at a few examples.

EXAMPLE #1: Avoiding stigma

"Recommended Terminology for Substance Use Disorders in the Care of Children, Adolescents, Young Adults, and Families"

A recurring theme in AAP Policy Statements is the non-judgmental or "avoiding stigma" approach.

For much of history, substance use has been viewed as a moral failing for which individuals themselves are to blame; however, as addiction became understood as a medical disorder, clinical terminology has shifted along with a growing awareness of harm of stigmatizing language in medicine.

As the first pediatric society to offer guidance on preferred language regarding substance use to be used among pediatricians, media, policymakers, and government agencies and in its own peer-reviewed publications, the AAP aims to promote child health by highlighting the specific context of infants, children, adolescents, young adults, and families. In this policy statement, the AAP provides 3 specific recommendations, accompanied by a table that presents a summary of problematic language to be avoided, paired with the recommended more appropriate language and explanations for each.

By deeming certain language to be "problematic", and actively denying any role of morality (either social or individual), the message is clear: i.e., there is nothing wrong about drug use as a recreational lifestyle, and if you suffer adverse effects from that lifestyle, it's because you are a victim. The AAP, by claiming to de-stigmatize language, and removing morality from the equation is actually promoting a different stigma, that of an individual with no 'agency', no ability to act in their own informed best interests. This erasure of individual responsibility goes directly to another common thread that runs among the AAP Policy Statements, the expanding definitions of childhood, and autonomy.

EXAMPLE #2: Blurring developmental lines

"Adolescents and Young Adults: The Pediatrician’s Role in HIV Testing and Pre- and Postexposure HIV Prophylaxis "

The HIV epidemic persists in the United States despite great progress in treatment and continued efforts to identify people living with HIV and AIDS. At year-end 2018, an estimated 1 173 900 people 13 years or older were living with HIV infection in the United States, of whom 47 800 (4%) were adolescents and young adults (hereafter referred to as youth) 13 to 24 years of age.

One technique that I see frequently in AAP documents is deceptive or misleading conflation of age groups when discussing certain issues. The linked article lumps HIV statistics together for ages 13 through 24, clearly disparate ages in terms of types of sexual activity, ability to consent, vulnerability to grooming/trafficking and sexual maturity.

EXAMPLE #3: Intentional distortion and misrepresentation of statistics.

"Firearm-Related Injuries Affecting the Pediatric Population"

"Pediatric Firearm Injury Mortality Epidemiology "

Firearms injury and fatality stats are frequently lumped together from ages 0 -19. This gives the impression that all children are at risk and supports a narrative of an epidemic of childhood firearm risk.

The [overall age 0-19] firearm injury mortality rate in 2019 was 4.15 per 100 000

When broken down into 5-year age groups (0-4, 5-9, 10-14, 15-19) a very different picture emerges. Clearly, firearms deaths in children and adolescents are predominantly among individuals in their late teens by more than 20-fold. (Graph adapted from CDC Mortality Data)

When you break it down by race and sex, the disproportionate mortality from firearms among black males aged 15-19 is even more stark, but discussion of moral and societal root causes for that must be inconvenient, as it is noticeably absent from AAP papers on the subject.

21 I would not so much call it moral abdication as shifting to a different set of morals. This might be acceptable, except that the morals they choose are generally destructive and never fixed. What was one banned is now compulsory, what was once required is now heretical. Its this shifting, never-predictable, and unfixed nature that makes it especially problematic.

Nobody knows where they stand, or what to expect. You cannot build a culture around jello, it has to have a structure to build upon, a skeleton.

Worse still, the people driving these changes are the most fringe, extreme, and demented of their lot.

27 Let me make it clear, the field of Pediatrics includes many principled, knowledgable, dedicated physicians who have, as a whole delivered high quality medical and surgical treatments for childhood diseases, and who mostly "stay in their lane", so to speak.. This is not a criticism of Pediatricians' medical expertise, rather it is a criticism of their blind deference to the AAP as an organization.

Agreed - I often say this country could use MORE discrimination. NOT against Blacks, Jews, etc... but against lowlife attitudes in general.

Children are wards, so people must remain children in some respects so that they can remain wards and do as they're told by the experts. But in other respects, the experts want something from the children and that requires some ersatz form of consent.

I think it's extremely sinister. You have no liberties when it comes to things classified as bad (smoking, gambling, etc.) and any liberty at all when it comes to things classified as good (sexual deviance, etc.).

48 If there is an overarching theme in all of this, it is that the AAP has, for whatever reason, sidestepped the moral aspects of child rearing in favor of a veneer of science and expertise, while diminishing the role of parents, extended family and community. They seem to be catering to a new constituency (policy makers and government agencies) and ignoring the old constituency (families), using youth as bargaining chips in the process.

Who owns children? Are they owned by their parents, or are they owned by the state? The AAP is clearly casting their lot with the schoolteachers, government drones, NGOs, media and whatnot firmly in the "state" camp. This pendulum does swing around somewhat, but the trend my entire life has been away from parents and toward institutions in terms of who controls children. This is just more of the same, and it is just as sinister.

I agree that it's very sinister. And it's being done with malice aforethought. Who gets to classify "good" and "bad"? Why the academy of experts, of course! They are the ones that know what you really need.

"Free will" is supposedly one of the greatest gifts bestowed upon us by our Creator.

These monsters would remove that gift, tear it into shreds before our eyes, and rub our noses in the remains.

Scope creep occurs (and let's face it, sometimes it's more like scope galloping), because you want to justify your continued existence. What struck me in your writing about the evolution of "standards of care" was its involvement in the recent Uvalde massacre. Was something similar operating at a subliminal level that caused the involved officers to stick to the script of their prior and/or extensive training (court-approved ???) instead of assessing the situation and following their own judgement and initiative.

And I think your clarification/re-emphasizing that you are discussing mostly an institutional problem is useful in that it applies to ..... well, seemingly almost *everything* now. Lots and lots of great individual pediatricians (and other specialties), law enforcement officers, on and on.

But sick/off the rails institutions and environments in which they must do their thing.

But the whole thing with teens (and younger!) being allowed to mutilate their bodies, and "doctors" participating (as opposed to kidnapping children to protect them, and physically disrupting any mutilation practices) - for "gender dysphoria" or related "reasons" is, to me, one of the most shocking and unbelievable collapses of medical ethics and adult responsibility in my lifetime (which is, needless to say, saying a lot).

Yes, the nuclear family, but also the intergenerational family. We ship our elderly to nursing homes, or they die alone in their own home. Yep. Especially when you consider the fact that psych units will physically restrain people who are superficially cutting their wrists. Gender dysphoria is a largely a socially sanctioned condition. It used to be treated by DBT and medication, now it's somehow a fact that they are "trapped in the wrong body", so the forearms need to be stripped of skin to create an artificial, and non-working "organ". There cannot be enough said about the hideousness of the practice.

Eventually those people will be forced out of their eternal adolescence. Mom and dad will finally get sick of it or will die. They will be unemployed with no plan and no money. They will get old and sick. It will end, eventually, because it must.

And when it ends, they will be utterly unable to cope with it or deal with it. This is why part of growing up was understanding that.

But when it comes to infectious disease, or at least ILIs, I now regard any physician as starting from zero in proving he/she actually is serious and scientifically based in their approach. One of the things I will give credit to Trump for is that he allowed key members of his administration to go through and start striking obsolete regulations. President Desantis needs to do the same.

Congress could legitimately go through the U.S. criminal and civil code and identify all sorts of obsolete, contradictory, and supplanted sections. Pass a law striking those sections, which would both make the code simpler and easier to understand/comply with and eliminate a way for the FBI/Deep State to entrap you with an arcane law that no one knows about or follows.

Every year they'd receive training on a whole host of OSHA type stuff (of course) but one that stood out was of a train engineer who came to talk to them about safety around train tracks. I don't know how it is possible, but the guy told them people do not hear trains coming. He said 'You never forget their face - they turn around in shock after it's too late to move."

My dream is a regulatory database that maps dependencies. I then order all agency heads to have a list of 10 regulations to be repealed on my desk no later than 15:00 every Friday. Those then get put into the system - a system to which only I have access - and those regulations are checked for any others that depend on them. Then those ten and any dependents are removed at the end of the day.

If that list does not appear on my desk, I will randomly select 25 regulations and give them the same treatment.

And the parents themselves are mostly allowing this to happen if not actively supporting it. I believe it started with the explosion of "parenting" and other supposed "expert" books on raising babies and children several decades ago. These books didn't tell you anything that you couldn't have learned from your mom through her experience. In that sense they weren't harmful. But then, like all industries, Big Parent fed on itself and created more and more content of more and more dubious if not sinister nature. Ask any parent today and there are literally hundreds of "expert" books offering all kinds of (dubious) advice on how to raise your kid.

It was a real tragedy because he was a really nice and pleasant person for the most part. You just never know what people are wrestling with on the inside, though. I can certainly relate.

This is true. I worked as an office manager for a guy who by all accounts should have been happy. Good marriage, three kids who were very successful, money, great standing in the community, etc. But he struggled and ultimately took his own life.

I had a buddy from Kent, WA who worked for the power company. Every year they'd receive training on a whole host of OSHA type stuff (of course) but one that stood out was of a train engineer who came to talk to them about safety around train tracks. I don't know how it is possible, but the guy told them people do not hear trains coming. He said 'You never forget their face - they turn around in shock after it's too late to move."

How can you not hear a train coming? Not sure. But apparently it happens frequently.

Well, one clue would be their website. If they've changed the background to rainbow colors this month, run. See what they say about vaccinations, especially HPV.

Also, make sure you look at the "confidential" questionnaires they hand to your teens before their appointments. If your teen doesn't really want you to see their answers, okay, there's some conversations for home as to why. But you can at least demand to see the blank form to see what questions are being asked. Hopefully, though, you have a good enough relationship so that you can see your teen's answers, and make sure you have a conversation after they meet with the doctor to see if there were any disturbing comments or questions that they didn't like.

The Newport Bridge is used fairly frequently by suicide jumpers and has been for years. But the incidents are almost never reported on due to the belief that publicizing them will lead to more copycats.

Yet if someone kills themselves with a gun it's big news if for no other reason than to demonize guns. We don't live in a world ruled by logic any longer.

Came from an affluent family, was in medical school, had a girlfriend, etc. Maybe smoked a bit too much weed in college, but no drug or other serious problems that I knew of. As a software engineer, I know that I can write 1000s of passing tests without knowing one thing that the software does. Because you're always testing your *expectation*.

Once it starts to interface with the real world, you can fail the same software all sorts of ways, because the customer/user didn't make the same assumptions.

In other words, you have to know what you're measuring.

To an extent they are discussed at the big national conventions, but in reality the AAP convenes working groups, "expert" panels, or such, and for some topics has standing committees who are charged with reviewing the medical literature and formulating the position papers, which ass I said, are generally accepted as authoritative.

I cancelled my membership in the AAP following sandy Hook when I saw their blatant misrepresentation of gun statistics. A few members have withdrawn, but not a huge number.

There was controversy about if the rescued could be charged for the time and equipment used in rescuing someone from a mine shaft. I don't know what they do now day's. My cousin had the miserable day of watching a guy hangliding up like 600 feet, when his straps broke, and he was dangling from his hands, until he couldn't hang on any longer. I remember driving slowly over a railroad crossing one time in my car, saw a light out of the corner of my eye, and a train was heading straight for me. I nearly had a heart attack and immediately hit the accelerator. It was moving very slowly, so I wasn't really in danger. It was also odd because this was in town and the gates should have been lowered and the lights should have been flashing. My best guess is that it had been stopped and was just now starting again (I have an amusing story about a different train incident where that happened).

That happens as well, although not in my examples. These four were all military kids separated by a dozen years. The murder was a young lady in college. She and her boyfriend were parked and were shot by a punk. The suicides were all teens indifferent locations at different times.

A few years back we were on a train in the U.K. when ,at a stop, the conductor came and said we would be here for some time as they had to clear the track of a suicide. He said it happens frequently and that we should be there for 30 minutes max.

307 How can you not hear a train coming? Not sure. But apparently it happens frequently.

When I first moved out to my new place, I could hear the train pass by (approx 25 times a day). As winter changed to spring it is really noticeable when I'm out in the yard goofing off. With the A/C on and as the air heats, I hear it less.

Last night, I went out for a swim. The railroad passes directly along the lake's dam. I swam down to the dam then returned. During that time, a a long train passed by within a few hundred yards and I didn't notice it until I paused long enough to get bearings.

You get used to the sub sonic vibration then soon the audible noise itself becomes part of the background. I hear the waterfowl, frogs and the diversity of nocturnal animals wandering around, but the train no longer registers unless one deliberately listens for it.

Was too young to know of Buckner's total career. Only later did I find out that he was a hell of a player, he had been subbed out for defense that postseason to avoid those plays, he had been playing hurt, and that wasn't even the final game of the series. Plus, I don't think he beats Mookie to the bag.

Yeah, more than a few moms want a baby because they love babies, but get tired or annoyed at them when they get a real one. And they keep growing up so they aren't babies any longer.

Every time I see a meme or social media post about how tough "adulting" is or how tiring and bad it is to raise kids, I want to punch them in the mouth. My mom had anemia and epilepsy and was so worn down by other illnesses she had to rest every day for several hours in the afternoon. She raised 4 boys and never bitched once about how it so HAAARRRRD and tiring.

Shut the hell up and do your job, stop complaining about your family. Stop running down your spouse in public. Stop complaining about being an adult. Grow the hell up.

It's a chemical that will glow under UV light in the presence of blood. Cops spray it to check for blood in an area that might have been cleaned up or to find splatter. Probably has other uses, too, but they don't show those other uses on TV.

Reminds me of some PSA TV commercial that Will Smith's squeeze did a few years back, trying to get young mothers to take their kids to the pediatrician on a regular basis. 413 Virginia law says that for the topics of drug abuse, mental health, STDs and something else I don't think it is abortion bu maybe it is - but for those topics, a minor OF ANY AGE is considered an adult when it comes to medical treatment. So the child - OF ANY AGE - can tell the care provider to keep privacy as an adult. So ntot only can the doctor not tell parents about it, they cannot even say they are treating the child.

So if a 4 year old decides to go to drug treatment, you can be kept from knowing about it. BUT YOU STILL HAVE TO PAY FOR IT.

Even though these studies were withdrawn, they continued to be cited and in the case of the ACE inhibitor one, continued to be used as a basis for policy.

(They didn't need to do that with the HCQ study, because they generated more sophisticated fraudulent studies that I can't talk to certain ex-friends about because I don't have a degree in the field. What's most bizarre is the guy who won't listen about the ACE inhibitor study for the same reason, as if if-I-had-a-degree a purported study with COMPLETELY FABRICATED data would suddenly make sense).
