Dr. Steven Smith: Hydroxychloroquine Works; "This is the beginning of the end of coronavirus"

This is not a study with a control group. It's a doctor giving the regimen to his patients and observing the results.

But the results are very encouraging.

How any flu kills you (well, usually) is by causing respiratory arrest. The alevoli which are supposed to let oxygen into your lungs and carbon dioxide out fill up with liquid due to inflammation. This liquid then keeps the lungs from exchanging oxygen -- so you go through the motions of breathing, but you can barely get any oxygen into your blood.

The last step doctors take to save a patient dying of respiratory arrest is to intubate him -- sticking a tube down his throat into his lungs to puff in air. Because most patients will fight against a tube down their throats, they have to be medically paralyzed -- or put into a medically-induced coma.

Obviously, this not a good state to be in. People who are intubated often die. I heard -- don't quote this -- that once you're intubated, the odds that you'll ever come off it are basically fifty-fifty. It's a coin flip as to whether you'll die.

This Dr. Smith did not have a control group, but he suggests that his patients can be divided into two different groups: those who arrived late to treatment and who therefore only got two days or less of hydroxychloroquine, and those who got in earlier and got the five days of hydroxychloroquine treatment he seems to recommend.

Not really an actual control group but there is a difference in treatment.

He says that of the patients he treated, only patients who had been on hydroxychloroquine for two days or less needed intubation. Anyone who was on hydroxychloroquine for five days or more did not need intubation.

He said zero patients getting hydroxycloroquine had to be intubated.

And intubation is really the sign that you have a good chance of dying. If that level of severe respiratory distress can be avoided by a cheap out-of-patent pill, then coronavirus just turns into a nasty but not very lethal flu.

And no one shuts a country down due to Sneezing and Running Nose season. It's only the risk of severe respiratory arrest that makes this scary.*

So he claims -- again, this is observational only; he did not divide his patients into control group and testing group and leave half his patients off the drug -- that based on his son's statistical analysis, it's close to impossible that it could just be random chance that only people who did only had two or fewer days of treatment would get intubated, and that no one getting five or more days of treatment would be intubated.

Not proven -- yet. But as more and more "merely observational" reports come in, the case in favor of hydroxychloroquine gets stronger.

And on the vaccine front: the University of Pittsburgh School of Medicine says that might have the vaccine ready.

University of Pittsburgh School of Medicine scientists today announced a potential vaccine against SARS-CoV-2, the new coronavirus causing the COVID-19 pandemic. When tested in mice, the vaccine, delivered through a fingertip-sized patch, produces antibodies specific to SARS-CoV-2 at quantities thought to be sufficient for neutralizing the virus.

An Israeli firm announced that it was ten days away from a vaccine.

One or more of these groups will nail it.


* Okay I've also heard that coronavirus is also killing via heart attack, but the heart and lungs are interrelated. I have to think that a lack of oxygen will lead to heart attacks, as the heart needs oxygen to work, too.

I think this is where the doctors think the HCQ works since it seems to inhibit the binding of the malarial worms to the red blood cells, which may explain in part how it is working here.

This is consistent with what the Hassidic doctor from NY indicated he was doing with his group. He encouraged *early* engagement with HCQ, Z-pak and Zinc for two groups:
- Elderly or with comorbidities, regardless of symptom severity.
- Anyone with breathing difficulty, regardless of age or comorbidities.

I think the words he used during an interview were "hit it early and hard."

He was using the full cocktail, not just (hydroxy)chloroquine, but it was a similar layout. Get in early, assume the worst with high-risk groups, and hit it.

Cause, effect.

People who are intubated often die because they have some horrific injury or condition that causes them to be intubated in the first place. It's like saying seat belts are linked to car wreck deaths because most of the people who die in car wrecks are wearing seatbelts.

An ET tube is a way of securing a patent airway. Yeah, it's no fun, but as far as I know it's on the safe side as far as medical procedures that involve shoving things into your body are concerned.

However, clinicians have pointed out that some COVID-19 survivors have developed acute respiratory distress syndrome (ARDS) -- a severe condition that, for the worst-affected patients, can last for the rest of their lives.

By itself it's not debilitating as long as I don't go high altitude hiking or extreme exertion. But more damage would not be welcomed. So staying put is a good move for me.

Now if we can get ahead of the medical people's need for masks we could start back to normal only with everyone wearing masks and the sick or diagnosed with Corona staying home.

This will be the two week period of pain Trump is talking about.

I would guess that's how long it will take to be able to have enough masks and to be able to tell everyone to take the malaria drugs.

163 This Dr. Smith did not have a control group, but he suggests that his patients can be divided into two different groups: those who arrived late to treatment and who therefore only got two days or less of hydroxychloroquine, and those who got in earlier and got the five days of hydroxychloroquine treatment he seems to recommend.

Not really an actual control group but there is a difference in treatment.

He says that of the patients he treated, only patients who had been on hydroxychloroquine for two days or less needed intubation. Anyone who was on hydroxychloroquine for five days or more did not need intubation.

I noticed that, too, and I think it is down to the context higher up. I assume this means that when they got in early with HCQ for 3-5+ days (a full course, based on the French study and other areas that have tried it), there were zero intubations. When they got there later, some still needed it.

A study is usually done comparing a new treatment with the current effective treatments. The use of placebos or non-treatment is not considered ethical, a point that Raoult has made to people calling his evidence

a) you have to prospectively randomize people to the two different dose regimens
b) it may be that i)low dose is not actively harmful and ii) high dose does nothing, so the untreated control is still important.

Sounds like what I had heard before. Chloriquine treatment slows down virus replication. If the virus has already filled you up, Chloriquine isn't as effective.

You give it to them early. If they are really sick, you are too late. That is why waiting five days for test results will not work.

Our trial thus far on HCQ + a macrolide antibiotic (we are out of Azitho, using others) has been moderately encouraging. Understand many patients were on vents before they got the meds, which is gonna skew stuff.

Two trials, one group for on vent before, one for not on vent. Three arms: no Tx, Tx w/ HCQ, Tx w/HCQ + antibiotic.

Sorting the drug induced cardiac and hepatic failure versus disease, can't yet. 1 case retinopathy and vision loss very likely drug related.

UNOFFICIAL consensus thus far with our team: If sick enough to be hospitalized, start meds ASAP. If stay home sick, no unless worsens to hospitalization.

The pool of zero or mild cases never diagnosed is likely to be HUGE and thus the case fatality rate will drop like a rock.

Checking antibody levels next year to estimate these minor cases will be fascinating. See also West Nile Virus immunity rates.

Sounds like what I had heard before. Chloriquine treatment slows down virus replication. If the virus has already filled you up, Chloriquine isn't as effective.

You give it to them early. If they are really sick, you are too late. That is why waiting five days for test results will not work.

Analysis - 'true' sort of.

My wife is treated for RA and Lupus (and other auto-immune) disorders. She had her (online) visit with her team this morning. They have had her on Plaquenil for quite some time. These are her Cleveland Clinic guys.

In a nutshell - they all told her to just live her life as she normally would but don't take unnecessary risks. The Plaquenil would help, they thought, but all of us and the general pop is going to be exposed (or not). And get the virus - or not.

If she shows severe symptoms I should bring her in. But the 'no contract with anyone else' regimen is burdensome and probably will not help her.

Just my $.02 and anecdotal at that.

A few years ago, someone did a study that used chochiline(sp?) as an anti-gout med, as it was used off label as an anti-gout drug. It worked, the study was approved by the FDA, so they PATENTED the generic drug, and jacked the price way up.

Will this FDA study allow a drug company to do the same thing? I doubt it, because the FDA is doing it.

411 I just recently took my brother to the hospital. He was treated like he had chinee lung ick, but he and I both knew it was due to a long term illness he has.

Fluid in his pleural lining was collapsing his lungs, and his heart was going crazy trying to get oxygen into the system. It is a finely tuned feedback system, and its no wonder it pushed people with weak hearts over the edge. I was concerned my brother would die from heart failure.

And I am currently outside Stanford hospital now as they drain it for a fifth time. Very quiet.

California needs to just start using masks and open up, and protect vulnerable populations like my brother by keeping those like him isolated.

428 Reading the comments on the NY Times on a story yesterday about hydroxychloroquine is fascinating. Basically breakdown of amateur epidemiologist talking about only clinical trials that are double blind are valid, comments about how orange-man-bad is preventing treatment, and a fair numbers of doc with anecdotal accounts of it working. Well there is an old saying that when you get enough anecdotes, it is call data.
What I a fascinated by is that there is a view that only double blind clinical trials that take years are valid. I would say how about we adhere to the Scientific method, and use that as a basis; ask a question, gather info, make a hypothesis, test the hypothesis, Is it working yes/no, analyse and draw conclusion. I would then add are the results repeatable. I think it is acceptable to say that when time is critical to substitute repeatable results from independent sources for a double blind study. The whole point is that individual researchers don't bias their own work. If you get repeatable independent results I would say that is better.

IIRC, there were two proposals:
- Spend half a billion dollars to stockpile ventilators to use as a reserve if a pandemic flu happened
- Draft up a plan to ration available resources if a pandemic flue happened.

They went with the latter. There are decent arguments to be made for either option. What pisses me off is, having made an eyes-open, considered decision at that time, they then went and demanded that the national ventilator supply be sent to New York. There's also the point about them wasting more than that much money on Mrs. DeBlasio's nonsense, but that's an entirely different thing.

IIRC, there were two proposals:
- Spend half a billion dollars to stockpile ventilators to use as a reserve if a pandemic flu happened
- Draft up a plan to ration available resources if a pandemic flue happened.

They went with the latter. There are decent arguments to be made for either option. What pisses me off is, having made an eyes-open, considered decision at that time, they then went and demanded that the national ventilator supply be sent to New York. There's also the point about them wasting more than that much money on Mrs. DeBlasio's nonsense, but that's an entirely different thing.

Removed "no risk" category and replaced with unknown risk to acknowledge that all persons in the United States are at some risk of COVID-19 given the increases in community spread throughout the United States.

620 As a heart doc I can tell you the "killing through a heart attack" story is a mixed bag.

Because we want to err on the side of detecting heart damage from newly blocked coronaries as early as possible, we have developed profoundly sensitive tests to detect small amounts of heart muscle enzymes in the blood.

Unfortunately though, with many severe illnesses the cardiac enzymes, the blood tests we do to detect myocardial infarction, turn positive. This is the bane of cardiology as we get called to see ever other elderly patient with sepsis from any source. Often when the dust settles we do angiography and find no high risk newish blockage to have caused this particular "enzyme leak."

Sometimes there is a new blockage, from plaque rupture due to the stress of the illness.

But most of the time, these enzyme elevations are not related to new anatomic blockage in your arteries. When your organs are dying, the heart is dying along with them. If we had supersensitive enzymes to detect very early minuscule cell death in other organs, they would also probably be positive when you are in septic shock or have very low oxygen levels from lung damage.

Because so many of the COVID 19 patients have positive cardiac tests, including enzyme leaks and abnormal EKGs, we are having to do a lot of triage. We don't want these patients in the cath lab because:
1. If their abnormal tests are due to illness other than a new blocked large heart artery, the trip to the cath lab benefits them not one whit. They get chest radiation and kidney exposure to contrast without any hope of benefit.

2. If I allow cath lab staff to be inadvertently exposed to a virus shedder, they will all be out on mandatory quarantine for 2 weeks. It doesn't take too many mistakes before you have destroyed an entire department's ability to respond to real MIs. And we are seeing quite a few big "real" MIs so we don't want people to have no emergency stent option.

Ok that was long and boring. Can you tell they've cancelled my clinic and I'm feeling useless today?

They had me on Propofol, the same stuff that killed Michael Jackson.

The family was worried sick but I was having nice trippy dreams.

628 428 Reading the comments on the NY Times on a story yesterday about hydroxychloroquine is fascinating. Basically breakdown of amateur epidemiologist talking about only clinical trials that are double blind are valid, comments about how orange-man-bad is preventing treatment, and a fair numbers of doc with anecdotal accounts of it working. Well there is an old saying that when you get enough anecdotes, it is call data.
What I a fascinated by is that there is a view that only double blind clinical trials that take years are valid. I would say how about we adhere to the Scientific method, and use that as a basis; ask a question, gather info, make a hypothesis, test the hypothesis, Is it working yes/no, analyse and draw conclusion. I would then add are the results repeatable. I think it is acceptable to say that when time is critical to substitute repeatable results from independent sources for a double blind study. The whole point is that individual researchers don't bias their own work. If you get repeatable independent results I would say that is better.

629 A doc in the Hasidic community(high number of cases because they refuse to social distance in any way) in Orange Country NY has had really good success. As of Monday he has treated 699 Wuhan virus patients and had only sent 4 to the hospital. I would say that is a rather good success rate. This will be down played for as long as possible as it ends the lefts chance to continue with their "not letting a crisis go to waste" campaign of tyrannical behavior and trying to stick more pork into "relief bills". It also means that Big Pharma loses a chance at a new expensive drug for treatment. They will still push for a vaccine but they were unable to come up with one for SARS and there are already 8 known strains of this virus. As you can tell I have a certain level of distrust, LOL. I do have to take certain meds and am not against conventional medicine but my experience as been one screw up after another by MDs. It's a case of if I knew now what I knew then I likely wouldn't be on any. Can't change the past.

633 I asked my daughter the nurse about this stuff. As it happens she has taken HCQ for years as a treatment for a chronic condition. Fortunate given her exposure to elderly sick people. She and her paramedic fella have 6 N95 masks they sterilize and rotate using with a handheld UV device. Wow.The national medical stockpile cupboard was bare throughout 7 years of Obama and 3 years of Trump. It is unhelpful to scapegoat either without finding out who the hell wasn't minding the store. If the only person responsible is the CEO of a 2 million workforce then can we lay off the rest of them?

634 There are numerous drug plants in the US and worldwide that could quickly start making and packaging HCQ tablets. That's the easy part. The critical supply chain issue is availability of the active ingredient, which is made in separate chemical synthesis plants. It's not easy to start up production of an API in a new plant. Hopefully the established manufacturers of HCQ are not all in China (as is the case for many older drugs) and can scale up their production quickly.
