Close them all. Close the VA. Create an insurance scheme that will provide the same cost of care but in the private sector, or even easier, just write big checks to United Healthcare or Aetna or whomever.... Then sell the buildings and the land and fire every single employee.
I don't know. Making it private, yes. And force accountability and a merit-based system. My dad, who has been in and out of the VA for years now holds a certain pride for the VA's. He enjoys the camaraderie and feels comfortable around other vets, where he could speak to them, sort of on the same level, where they all understood. He has a feeling of worth and pride amongst his fellow veterans. Someone else I know, although younger, felt the same way. I seeing, after Trump, and other uphill battles in life, that things can change is the will is there. Trump has been working on the VA, and it's only just begun. I think our Vets are special and deserve that extra amount of care (not just health) but in respect and camaraderie. Many are allowed to go outside the system now, which is good, but I hope they can fix this. I've been living in VA in the past year or two, and there are many good doctors and nurses. I think it boils down to Big Gov facility, no merit based system, impossible to get fired, and the leadership. There is a huge VA in my old home town, Thery were supposed to close it a few years ago but they didn't know what to do with patients. And those patients were largely alcoholics that needed to be warehoused instead of treated.

But I am a bit of a heretic on the VA. Because I would advocate cutting the budget and benefits. The complaints in the VA are around the medical system. I think that it provides the best medical care that you can get for our war shattered vets. No better care possible for amputees and trauma victims, I don't think. The problem comes with chronic medical problems and issues like alcoholism and drug addiction. VERY expensive and resource intensive to treat. A veteran is anyone who has served 180 days or more in the military and discharged honorably. I support what we do for our war injured veterans - we can't do enough for them. There are other veterans - veterans like me - who never got shot at, not injured in the military - and we all have a claim on the VA. We might be very far down on the list, but we are on the list. I don't think spending a couple of years painting rocks at Ft. Riley should entitle you to lifelong healthcare. Yes, I get it, someone who served a 2 year hitch at Ft. VA healthcare ranges from world class, to absolutely horrible, with everything in between.
I know a case where an internationally renowed professor worked at his prestigious private clinic seeing millionaires and rock stars three days/week, then saw VA patients 1 day/week. . .because he wanted to. Only difference in care was that he was limited to what he could prescribe the VA patients based on what was available at the VA formulary. Some really specialized testing wasnt' available, but he was good enough that he could just make do. Then there are the doctors so bad they can't or won't work anywhere else Anyway, I agree. I'm a vet and I am disabled but not because of my time in service (genetic thing). I qualify to use the VA because I deployed into DS (Iraq 1.0). I thank god that I have other insurance (medicare) because using the VA system has been a fail for me. I get it I have a rare disease that non specialist DRs have never seen, most have only heard of in school let alone ever had to treat. But the VA is useless to me. I will never go back if I have any other options.

I went back to work after retiring mainly for the insurance. After 30 years my ankles and knees are shot. I will not subject my health to TRICARE or the VA. I go to VA for hearing aids only.

Somehow when the papers were filed with whoever the hell you filed with back then, it was accidentally spelled Lee's Summit instead of Lea's Summit. Actually healthcare is not very complex, it has been made very burdonsome and expensive because of government meddling.
I recently took my 6 month old for a check up and to get vaccines. My EOB reads for the less than 20 minutes of actual service... Charges: $1,200.00 Allowed: $743.00 Provider Responsibility: $476.62 Deductible: $15.00 Member Responsibility: $25.00 Amount Paid to provider: $717.00 Nothing adds up, so either the Dr is over charging, or he's getting stiffed. The Dr either is going to agree to the amounts specified by the Insurance company or try to bill me, typically I don't see any additional bills but that hasn't always been the case. My mom worked for the VA at Hines for from the 1950's thru the 70's . She was a nurse and lab tech. She always felt it was her duty to give the patients the best of care. Of course it might have been because she was a WWII vet herself

I don't have any figures but I would guess that well over 50% of VA patients are one time enlisted soldiers who never left the States or were never injured while on active duty.

That sounds like an argument for establishing vets-only social clubs, (which I'm fine with). I have no faith that even Trump will be able to fix the broken VA culture. The sacred obligations we have to them are simply incompatible with the government's lack of a work culture and accountability. And I say that as a Fed.

It's impossible to explain in an Ace blog after spending so much time in one VA. Not all are the same, I understand. Mental care is also crucial. I drove in one morning after a young vet had committed suicide. I am sure I saw him on one of the previous Sunday's at the VA chapel. And I am friends with the Father of the Catholic services there. He had been up all night attending to this boy and his family. It is difficult to explain the feeling of family and understanding towards one another that I think is key among vets. Like I said above, it can be privatized, and have merit-based and accountability systems put in place, but there is more to care then just the bed and meds. The trauma involved can be devastating. If you haven't seen the miracle of volunteer and other types of care-givers, it is impossible to describe. I've got to get back to work. Subsequent visits are 30 minutes plus as required. She has been called promptly with test results, she can either skype or call in for a conference call with both the doctor and associated practice professionals that supervise her care, such as the nutritionist when her blood sugar spiked after dropping within normal limits from the diet help she was given( she didn't realize she couldn't eat unlimited fruit during the day). I've been impressed. And yes, she is on Medicare, and pays additionally for part B, etc. in addition to the $150 a month. The amount of attention and care she has received has resulted in normal blood pressure and normal blood sugar, reduced weight even though she is in a higher risk category due to family history and genetics. I just wrapped up 3 months of ER and Dr visits, including CT scans, xrays, MRI's, various blood tests, to find I did not have pneumonia, pleurisy, bronchitis, ascites, pleural effusion, costochondritis, marfan syndrome, but to find the correct dx of zoster sine herpete... internal shingles. 8 different doctors, 5 different facilities,12 weeks. Overtreated with antibiotics I did not need. Overused emergency and urgent care as private practice specialist are unavailable for 5-6 weeks for appointments.

At this point, with 5 year service requirements and a dwindling military, it may be that giving 5 years of service to the military is worth paid healthcare for life.
Whether it is or isn't, it strikes me that the VA system is a lousy way to deliver the healthcare. However, from what I have heard, the VA offers the intangible benefit of connecting suffering veterans together. However, the default is that if you have any kind of insurance, e.g., employer-provided, Medicare, etc, the VA bills your insurance. The amount of VA care that is actually given at zero cost to veterans is pretty minimal, most of them (or their insurance provider) still get a bill. If I were designing the system from the ground up, I'd have the government pick up the tab for wounded combat vets' combat-related injuries (including mental health). The big problem with the VA is that there are competing views of what it is supposed to do.
Veterans think the VA is supposed to provide them with medical care. The VA and the rest of the government think the VA is a jobs program. It's purpose is to provide people with hi paying employment. The VA is the problem with the VA. They lost the original submission (despite me receiving 3 confirmation emails that they had it). There are limitations to what the VA provides based on the nature of one's service. When I got out of the Navy after six years, I got a free cursory exam and IIRC a couple of years of discounted (not free) care, because I had not served in a combat zone and had not been injured in the line of duty (at least, not enough to warrant medical care). After I deployed to Iraq with the NG, I came back to full VA coverage. However, the default is that if you have any kind of insurance, e.g., employer-provided, Medicare, etc, the VA bills your insurance. The amount of VA care that is actually given at zero cost to veterans is pretty minimal, most of them (or their insurance provider) still get a bill. I've been impressed. And yes, she is on Medicare, and pays additionally for part B, etc. in addition to the $150 a month. The amount of attention and care she has received has resulted in normal blood pressure and normal blood sugar, reduced weight even though she is in a higher risk category due to family history and genetics.

REWRITE... Vets with injury's connected to service and vets with wartime service deserve good prompt care. Vets not injured in service and that served outside of war should not be able to use the VA. The standards connecting service to injury need to be reasonable but low.

Of course the folks sitting on the firing line (aka the table) we not the same ones that we in the firing line the last time so they could only give the Lance Corporal salute (shoulder shrug and I don't know sir/ma'am and we will get back to you). I will say that I've never experienced anything negative other than a far reaching appt which they outsourced me to a specialist since I had to wait more than 30 days in all my visits and procedures. But to get full, free, healthcare you need to retire from the military (20 years), and even then, if you have private insurance, the VA can / may bill your insurance for services provided. Now the "service related injury" thing seems to be pretty broad to me. As I mentioned up thread, I have a few friends that are seemingly, well, scamming the system a bit. You get a "percentage disabled" rating from the VA, and the goal for many is to get to 100% (full disability.) You can "stack" disabilities to get there. So you can claim reduction in hearing (impossible for the VA to disprove) for say 10% disability, irritable bowel syndrome for 15%, depression for 10%, anxiety, etc. etc. etc. with the goal of getting to 100%. They are subject to an annual means test, I believe. So if you qualify for healthcare, based on low income, and you go out and get a job, once your income hits a certain level, you can be thrown into a higher copay bracket, up to and including having to pay every cent of your care (either through your own insurance, or out of your own pocket).

My Father-in-Law was career Air Force. He started with the AAF, back in WWII. He went to a Chicago area VA hospital for a checkup every year. They told him he had anemia, every year.

After I had my heart attack scare the VA did a stress test on me and it showed significant blockage. I went to the VA in Dallas and they said I had to try a three year medication therapy before they would do a heart cath. I said oh hell no and the wife who works at the VA bought Blue Cross during the open season. Went and found a Civilian Doc and he did my cath and fixed me up in less than two weeks. In the meantime I had called the VA and cancelled all of my heart clinic appointments. The head Doctor for that clinic called me at home asking what was going on and why did I cancel all of my appoints and that he was concerned about the blockage, told him I was OK and thanked him. There are some very good Doctors there but they are constrained by the VA system and they can't go outside the box or change anything in the treatment hierarchy. You can "stack" disabilities to get there.
So you can claim reduction in hearing (impossible for the VA to disprove) for say 10% disability, irritable bowel syndrome for 15%, depression for 10%, anxiety, etc. etc. etc. with the goal of getting to 100%. The DAV National Service Officers that work on your behalf have a book thicker than the bible and there may be a list of things listed that add up to 175% but we added collectively in the good book may only equal 60%. One of my older brothers thought he was getting good treatment from the Dallas VA until he called an ambulance to come get him. They took him straight to Parkland and they diagnosed him immediately for something completely different than the VA had been treating him for two years. He was down to just over 100 lbs when that happened. He looked like a walking skeleton. Glad to report after private treatment (including surgery for a blocked intestine) he is gaining weight at 155 lbs, looks and sounds like he did 20 years ago and so glad to be alive and away from the VA. This maybe an unpopular opinion but I do not think EVERYONE in the armed forces deserves to be treated as some god. I have access to the local AFB (safely here in the good ol' US of A) and I see guys in fatigues taking water samples and otherwise not doing much so forgive me for not getting worked up that they aren't getting the world's finest healthcare or they have to wait like I do. I guess there's a reason they call it the Chair Force. Really, what they're doing is not hard but I'm supposed to kiss their @$$ for doing it. I wouldn't completely eliminate the VA. Where they have attained expertise in treatment and rehabilitation of things such as lost limbs, etc., things you don't normally see or see in much lower frequencies in non-military settings, I would retain them in specialized centers. But there's no need for veterans to have to go to the VA for treatment of conditions that your standard hospical/medical center is competent to treat.
