Halp needed!

Okay, y'all. Time to bust out with those mad code skillz. I'm looking for a small submission form for Memeblog to make it quick and easy for people to submit their stuff. It would sit in the sidebar and the results would be emailed to a preconfigured address. The form would have four parts:

Field 1, text fill-in: Memename
Field 2, text fill-in: URL
Field 3, text fill-in: email
Field 4, drop down box: Type (Options would be Submission Reminder, Posted, Introduction, etc)

Fields 1 & 4 would be mandatory. Either 2 or 3 would be mandatory (both would okay but we need at least one of them for sure).

Can anybody help with this?

1 You have the tools to do this in C-Panel. If you need the HTML for the form I can pop it in the comments.

For form validation you can use Javascript to ensure the form has been filled in... or someone needs to write a super crazy PHP form handler with validation and postback ... I think JavScript is the way to go. I've got a load of validation stuff so can email it over

2 Camilla Belle in a 2009 dress by christian louboutin outlet Alexander McQueen, listed among "100 Best Dresses of the Decade" by InStyle Magazine.
McQueen's early runway collections developed his reputation for controversy and shock tactics (earning the title "l'enfant terrible" and "the hooligan of English fashion"), with trousers aptly named "bumsters" and a collection entitled "Highland Rape".McQueen was known for his lavish, unconventional runway shows: a recreation of a shipwreck for his spring 2003 collection; spring 2005's human chess game; and his fall 2006 show "Widows of Culloden," which featured a life-sized hologram of supermodel Kate Moss dressed in yards of rippling fabric.
McQueen's "bumsters" christian louboutin outlet spawned a trend in low rise jeans; on their debut they attracted many comments and debate. Michael Oliveira-Salac, the director of Blow PR and a friend of McQueen's said that "The bumster for me is what defined McQueen." McQueen also became known for using skulls in his designs. A scarf bearing the motif became a celebrity must-have and was copied around the world.
McQueen has been credited with bringing drama and extravagance to the catwalk. He used new technology and innovation to add a different twist to his shows and often shocked and surprised audiences. The silhouettes that he created have been credited for adding a sense of fantasy and rebellion to fashion. McQueen became one of the first designers to use Indian models in London.
McQueen also designed a range of christian louboutin outlet dresses under the name of "manta", priced at around £2800. The line, named after the manta ray, was inspired by a holiday McQueen took in the Maldives in 2009. The designs have been worn by various models and celebrities, including Lily Cole.
Givenchy appointment
The president of LVMH, jimmy choo outlet Bernard Arnault, caused a stir when he appointed McQueen head designer at Givenchy in 1996, succeeding John Galliano. Upon arrival at Givenchy, McQueen insulted the founder by calling him "irrelevant". His first couture collection with Givenchy was unsuccessful, with even McQueen telling Vogue in October 1997 that the collection was "crap". McQueen toned down his designs at Givenchy, but continued to indulge his rebellious streak, causing controversy in autumn 1998 with a show which included spraying paint over white cotton dresses and double amputee model Aimee Mullins striding down the catwalk on intricately carved wooden legs. McQueen stayed with Givenchy until March 2001, when the contract he said was "constraining his creativity" ended.
Accomplishments
Some of jimmy choo outlet McQueen's accomplishments included being one of the youngest designers to achieve the title "British Designer of the Year", which he won four times between 1996 and 2003; he was also awarded the CBE and named International Designer of the Year by the Council of Fashion Designers in 2003.
December 2000 saw a new partnership jimmy choo outlet for McQueen, with the Gucci Group acquiring 51% of his company and McQueen serving as Creative Director. Plans for expansion included the opening of stores in London, Milan and New York, and the launch of his perfumes Kingdom and, most recently, My Queen. In 2005, McQueen collaborated with Puma to create a special line of trainers for the shoe brand. In 2006 he launched McQ, a younger, more renegade lower priced line for men and women.
McQueen became the first herve leger outlet designer to participate in MAC's promotion of cosmetic releases created by fashion designers. The collection, McQueen, was released on 11 October 2007 and reflected the looks used on the Autumn/Winter McQueen catwalk. The inspiration for the collection was the Elizabeth Taylor movie Cleopatra, and thus the models sported intense blue, green, and teal eyes with strong black liner extended Egyptian-style. McQueen handpicked the makeup.
Company
By the herve leger outlet end of 2007, Alexander McQueen had boutiques in London, New York, Los Angeles, Milan and Las Vegas. Celebrity patrons, including Nicole Kidman, Penélope Cruz, Sarah Jessica Parker and Rihanna, have frequently been spotted wearing Alexander McQueen clothing to events. Bj?rk and Lady Gaga have often incorporated Alexander McQueen pieces in their music videos.
By wearing his designs, celebrities such as the herve leger outlet above mentioned have further increasesd the notability of the McQueen brand. It almost seems as if he created these extravagent designs for those "out there" artists like Lady Gaga, who can appreciate the crazy and fun spirit that go along with the peices. The shoes that Lady Gaga was featured wearing in her "Bad Romance" video which was released in 2010 almost defy nature in itself, by portraying a whole new, very unrealistic shape of a foot. He went beyond the natural and normative of fashion with pieces such as these, and by doing so became a famous trend-setter.

Hollywood’s hottest celebrities have long had a penchant forChristian Louboutin’s signature red soles. In fact, the fameddesignereven made a pair specifically forBlake Lively, who is inarguably one of his biggest fans. Last night, at the 2011 MTV Movie Awards in Universal City, California, formerly retired actress Amanda Bynesgraced the red carpet in askintight Herve Legerbandage dress — but it was her Louboutin Change of the Guard pumps that had us doing a double-take! This wasn’t the first time we’ve seen the fringed footwear. It turns out the Kardashian sisters as well asLaLaare all proud owners of the electric blue version of thisdesigner shoe!Back in February, for husbandCarmelo Anthony‘s much anticipated debut as a New York Knick, LaLa stayed stylish on the sidelines, posting a TwitPic of her “game shoe” — the Change of the Guard sandal, which conveniently resembles the Knicks’ team color! Then, on Apr. 7,Kourtney Kardashianwore the samedesignershoeswhilegallivanting around LA.Just three days later, little sisKhloepaired the pumps with matching jeans while enjoying dinner with hubbyLamarat paparazzi playground Katsuya. If they were a normal family, I’d say that maybe they shared the shoes, but after we got a glimpse intoKhloe’s gargantuan closet, chances are they both happen to own the same pair. Ah, the sweet life of a basketball wife!
Depending on the rest of the outfit, thesedesigner shoescan really be a hit or miss. I love what Kourtney did with them, but I wish Amanda Bynes would have gone a little less bold, especially considering the basic, recognizable (not to mention slightly overused) design of the Leger she wore. Regardless, these are truly an example ofLouboutin’s impeccable craftsmanship, and we are always waiting for his next collection!
