I have an inside on this, my nephew is an air traffic controller (he learned it in the air force, then retired and works at smaller airports). He says that they are understaffed and overworked, heavily. Its a very high skill job that not a lot of people can do, and they just do not get a lot of applicants. Pays pretty well but its long hours. He has to apply like 6 months in advance for vacation time and scramble to find people to cover.

But I must stress this is only ancillary to the actual investigation that is being carried out by the NTSB.
Inadequate staffing, if true, will get cited along with all the other factors in the mid-air collision. There is usually a chain of failure, break one link in the chain and there is no accident. The school to be an FAA controller is located in Oklahoma City. Even if you are coming out of the military as an air traffic controller the FAA rule used to be you went to Oklahoma City.

Based on the radio chatter, the helicopter pilots and air traffic controllers knew the plane and helicopter were approaching each other. The traffic controllers told the helicopter pilot to pass behind the plane and the helicopter pilot fucked up.

To be fair, from the UHF and VHF ATC convo's I heard -- wasn't really ATC's fault here.
The helicopter was supposed to hold until after the CRJ passed, then go behind it. Somehow it flew directly in to the plane. The biggest fucking question is why is routine helicopter traffic intercepting the glide slope for jets at a major US airport? Trump is a loose canon, no doubt (thanks Captain Obvious, I know) but for those wringing their hands today about him bringing up Peter Bootyjuice and DEI in the FAA, I think it more than likely (and common) that Trump knew going into that press conference about the short-staffed control tower, one guy doing the job of two, and all the applicants turned down for jobs. Funny true story. My wife once worked at a company in Herdon, Virginia, which is right next to Dulles Airport. Their fax number was the old fax number for the FAA, who had changed their number but hadn't given out the new number, at least not to everyone. Wife would keep getting flight plans faxed to her. Finally, one time a pilot called to confirm that the "FAA" had received his flight plan. My wife set him straight. The pilot responded, "You mean, this isn't the FAA? Uh oh."

Based on the radio chatter, the helicopter pilots and air traffic controllers knew the plane and helicopter were approaching each other. The traffic controllers told the helicopter pilot to pass behind the plane and the helicopter pilot fucked up.

Twice the ATC warned the helicopter pilot about the jet, twice ATC asked if the pilot saw the jet and the pilot replied yes, I can see it and I am requesting visual separation both times. I was a Blackhawk helicopter crew chief in the Army. I was even a Flight Instructor. This means that I trained Crew Chiefs and ensured that they completed all training annually to maintain their flight ratings. One massive responsibility we had was to be the eyes for the pilots. We handled airspace obstacle avoidance and communicated potential risks to the pilots. Quite often we would train as a flight of 2 or 3 birds flying in formation. It was my job to have my head out the window and tell the pilots that the aircraft behind us was "staggered right at 3 discs". (We measured close distances in terms of the diameter of our rotor discs). I can tell you after doing this for hundreds of hours, even when you know EXACTLY where a Blackhawk is, and you have night vision goggles on, it is EXTREMELY hard to SEE the aircraft. These birds are designed to be hard to see at night. The red and green lights on the side get lost in the lights of the city below. The only "lights" on top of the aircraft are called "slime lights" because they are a very very very dim green. Incredibly difficult to see. If you are above the helicopter, even if it has it flood light or spot light (2 different lights) on, underneath it, it is still hard to see the bird because all of that illumination is below the airframe. Another thing people should know is just how busy things can get on the aircraft. Pilots are talking to each other about what they are observing on the instrument panels. This means neither are looking outside the aircraft. The crew chief might be conducting a fuel check, where we would also be looking up into the cockpit at the fuel gages and the clock. This CAN lead to moments where all 3 people on the aircraft are all looking inside the aircraft. It's not supposed to happen that way. We are supposed to announce when we are "coming inside" or are "back outside" the aircraft. But that doesn't always happen. Also, in cities like DC, the radio traffic is constant and can make it hard to filter out what is important for you to listen to. Checking instruments, doing math, reading checklists, and listening to multiple radios all at the same time is HARD. Mistakes happen. The plane was, as far as we can tell, exactly where it was supposed to be. The helicopter seemed to have jumped the gun. My guess is the pilots were watching the wrong plane and thought they were cutting behind the CRJ. But we'll know for sure in a year or so when the NTSB released the full report. I agree keeping NTSB away from having the power to force changes is a good thing.
Then there is the matter of the DC-10 and their outward retracting cargo doors. US DC-10 suffered a blow out and had major control issues, but pilots managed to land it. NTSB recommended changes. The FAA instead worked a sweetheart deal with MDD to avoid an Airworthiness Directive. And guess what a Turkish DC-10 flying out of Paris suffered the same type of blow out but on that plane the cabin floor collapsed destroying the flight controls. Between No-Fly Air Space around Federal Buildings to flights in and out of Reagan, to Ft. Belvoir, to private aircraft, etc.... One big question is were the Helicopter Pilots on NVGs and concentrated on the ground ? You may hear ATC tell you that you are enter the landing glide path into Reagan and on a collision course, but again if the Airliner had it's landing lights on it could affect your nighttime vision with NVGs. Was listening to someone with experience in aviation and they were saying that when the helicopter pilot said that they were aware of the plane when asked by air traffic control they were probably looking at a different plane.

Twice the ATC warned the helicopter pilot about the jet, twice ATC asked if the pilot saw the jet and the pilot replied yes, I can see it and I am requesting visual separation both times. Correct if I am wrong but I this think means he is assuming all the responsibility to avoid conflict.

Typically someone is always, always, always looking outside the helicopter because outward visibility other than forwards, sucks. I do not know if the VH-60 had TCAS installed. But assuming it did and Lirio is correct, below 1000ft TCAS will not command evasive action. TCAS will only alert you. I aced the psych exam, had no legal issues, finances in line, excellent references including an Uncle who was running one of the Florida FBI offices at the time. Even with no passengers, the crew chief does a lot of safety checks and such so he's typically on flights regardless. I agree keeping NTSB away from having the power to force changes is a good thing.
Then there is the matter of the DC-10 and their outward retracting cargo doors. US DC-10 suffered a blow out and had major control issues, but pilots managed to land it. NTSB recommended changes. The FAA instead worked a sweetheart deal with MDD to avoid an Airworthiness Directive. And guess what a Turkish DC-10 flying out of Paris suffered the same type of blow out but on that plane the cabin floor collapsed destroying the flight controls. I no longer fly as much as we used to as in once or twice a month for over a decade. We are flying in two weeks and I will hope for the best, as always.
To sum: before and since 9/11 I have no fear of flying and never have. Always prepared with my heavy travel purse to whack on bad guys as we plummet. In 2025 I will not be afraid to fly as we have two trips lined up already. I will, however, ask to see the air traffic controllers and if I see a dwarf or psych patient I will call the WH. I had a cousin who was schizophrenic and paranoid, among other things. As a child, he was a lot more shy than me, altho he was older and I was sort of bossy. Classic schizophrenic symptoms showed up in his early 20's. I don't think I'd want to ride in a car with him back then (altho I did pre-diagnosis), much less put my family in his care at 30,000 feet. Oh I meant the federal government, sorry if that wasn't clear! But yes, state and local leaders still fund and support this all on their own. Hopefully though, less federal money or grants flowing freely will make decorative feel good projects dry up, and then the state or the city will also have to tighten their purse strings. I was working construction at Hobby Airport in Houston may years ago (1987 era) and was invited up to the control tower room. Three guys sitting or standing casually looking at dizzying amounts of blinking lights with numbers floating by, wearing headphones with mics, talking 90 miles and minute into their mic and occasionally to each other. One of them sez "here, you wanna hear this?"
It was a cacophony of chatter! One guy is monitoring planes in the air near the airport, one guy is guiding everything one the ground and one guy tracking planes way the hell out there. And all three are are listening to all of it. Flight controllers at work have to be the most aware, collected, calm and intelligent humans ever created to do that job. I always thought there should be software where you can enter the symptoms and get a response that would lead you to ask more questions and those answers would be inputted , repeat as many times as needed until the list is one or two diagnoses.

I think TCAS is disabled below 1000ft for simple reason that TCAS avoidance instructions are coordinated between the two conflicting planes and is an instant announcement of "climb, climb" for one and "descend, descend". Pilot in right seat, amidst doing what he was supposed to be doing, he might have been the PIC flying the plane to the landing you know. And you expect him to peer through the back corner of the windscreen and probably see through the wing to spot the UH-60?
