Drug-Based GAINZZZ?

First, some nonsense: "The Relentless Reality of Anti-Fatness in Fitness."

These biased "Fitness Experts" who continue claiming that being greatly overweight is bad for your health and longevity! Haven't they read The New Science (TM)?!

A "seasoned athlete," who is also grossly obese, relates this "comical" story of a gym fitness pro trying to instruct her in how to exercise.


Jaffe laughed as she recounted the story to me--and it is almost comical, imagining this seasoned athlete staring down at a two-pound dumbbell while a trainer tries to "teach" her what a bicep curl is. The session continued as such, Jaffe recalls, with her asking about functional exercises and the trainer instead demonstrating a squat. "I kept having to reiterate, 'I know what these things are. I want you to give me something new, so I don't get bored.'" Though she'd been very clear on her goals, the trainer seemed to have another one in mind:

"You do know that you need to eat fewer calories than you burn," she said, "in order to lose weight."

"What makes you think I want to lose weight?" Jaffe asked. "I didn't put that on my intake form." The trainer said she'd assumed as much because of Jaffe's "curvier" physique. One last time, Jaffe--who was then a size 16--explained that she was there to support her endurance training. It was literally there, in writing.

The trainer seemed flummoxed. "So you did those things you wrote on your form?"

This is the woman who insists it is "comical" for a fitness pro at the gym to assume she needs instruction about how to exercise:

How dare you assume I need coaching! I'm obviously a fitness fanatic!

Though bizarre, Jaffe's experience is hardly novel. Of the dozens of people I spoke with for this story, few could identify one specific incident of anti-fat bias in a fitness space that stood out more than others--because incidents like this are the rule, not the exception.

Though the term "fatphobia" only went mainstream in recent years with the rise of the body positivity movement (and has already fallen out of favor for some), anti-fat bias has been a part of white America for centuries.

To make a long, complex story very short: People were suddenly interested in quantifying health, and life insurance companies promoted the use of weight tables (though they differed greatly from company to company) to do so. In a matter of decades, fatness evolved from a moral issue to a medical one.

Fatness was always a medical issue. Doctors in the seventeenth century noted that fat people tended to die young. Doctors encouraged people to diet in the 1800s, when it was a "disease of the affluent."


There's another concept we didn't really hear about in the '50s that gets a lot of play today: obesity. The Centers for Disease Control and Prevention (CDC) defines obesity as "weight that is higher than what is considered healthy for a given height" and positions it as being in the middle of a spectrum ranging from "overweight" to "extremely obese." Between 1960 and 2012, the rate of obesity in U.S. adults increased about 20%, according to the CDC. (Though, depending on race and gender, there's significant variation in both the statistics and the volume of data.) Concern over this matter escalated when, in 1998, the NIH tweaked its Body Mass Index classifications, lowering the cutoff for "healthy" weight from 27.3 (for women) or 27.8 (for men) to 25. Overnight, nearly 30 million Americans went from a-okay to soft, sick aberrations--symptoms of the epidemic ravaging this great nation.

With all due respect: It's not people who are borderline obese who are really at risk, and of course it's not the people who are borderline obese who are shrieking that "obesity" itself is a made-up concept invented by white supremacists.

It's the people who are severely obese and would be diagnosed as such under any plausible metric.

In good news for the obese: You know how fat celebrities keep losing a bunch of weight? Adele? Rebel Wilson?

Even Mindy Kaling dropped a lot of weight, and appeared to be actually... thin.

And then they say it was just diet and exercise?

Well, yeah, like the "Marvel Method" of bulking up actors to look like men who, coincidentally, appear to have been on a one-year cycle of steroids, they're not just relying on diet and exercise.

They're taking a new class of drugs which appear to actually work. Weight loss drugs Ozempic, Wegovy, and Mounjaro are flying off the shelves so fast that there are national shortages.


In the past five years, several new drugs have been brought to market that could lead to a profound, if not revolutionary, change in how health care providers -- and the public -- view weight loss.

Three drugs in particular -- sold under the brand names Wegovy, Ozempic, and Mounjaro -- have shown remarkable effects on weight loss in clinical trials.

While Wegovy is approved by the Food and Drug Administration for weight loss, the other two medications are only approved as treatment for type 2 diabetes, though they do also contribute to significant weight loss.

This has led to increased demand for these drugs, causing a shortage of Ozempic and Mounjaro in early 2023 that threatened the availability of these drugs for diabetes patients.

Globally, more than 650 million people are obese, and by 2030, an estimated 1 billion adults will be obese. This makes obesity the most prevalent chronic disease in the world.

As an exercise science scholar, I have spent more than 15 years studying the effects of diet and exercise on the human body, primarily focused on body composition changes such as gaining skeletal muscle or losing body fat, or both.

While these medications hold promise, they are not wonder drugs. In my view, they warrant much more research before they become the basis for a new weight management protocol.

Ozempic and Mounjaro are FDA-approved for the treatment of type 2 diabetes, which affects more than 37 million people in the US.

On top of that, nearly 100 million people are considered prediabetic, meaning that together over 40 percent of the US population suffers from an inability to properly manage blood sugar.

Obesity and type 2 diabetes are closely related conditions, so much so that some experts are now calling the two conditions "diabesity". These dire figures are what prompted pharmaceutical companies to search for new weight loss treatments.

Fat people are not fat because they're particularly lazy. They're fat because their bodies don't properly regulate their blood sugar. Their bodies would probably be pretty healthy during periods of food scarcity, but, of course, the western world doesn't see much food scarcity. It sees mostly food overabundance.

And fat people's hormones just aren't set properly to deal with that.

The drugs are designed to readjust the body's hormones to liberate blood sugar from fat, and store less glucose as fat.


Wegovy and Ozempic are a class of drug known as a semaglutide. Since people with type 2 diabetes are unable to properly manage blood sugar, injections with this class of drug help the body release the proper amount of insulin after eating by mimicking a naturally occurring hormone called GLP-1.

At the same time, semaglutides also reduce the release of the hormone glucagon, which works opposite of insulin to increase blood sugar. In non-diabetics, these two hormones work together to maintain normal blood sugar levels.

But since this mechanism is not functioning properly in diabetics, which leads to high blood sugar, semaglutides help to control blood sugar and also reduce appetite.

I don't know if the drugs work. Anecdotally, I've heard they do; this article calls the results "staggering" in some cases.

Are there long-term health risks associated with the drugs? I don't know that either. I do know how Dr. Atkins responded to claims that the Atkins diet might have long-term health risks: Obesity itself is a very well-documented health risk. One can worry about hypothetical unknown risks, but in the meantime, the known, proven risk must be treated.

I'm not endorsing these drugs. I just know they're getting more and more popular and it seems I should mention them.

Natural GAINZZZ are the best GAINZZZ, of course.

But maybe the perfect shouldn't be the enemy of the merely good.

I mean... SOME of them are fat because they are especially lazy, I have known a lot. Some are fat because they just eat everything and have no self control. Some are fat because of a chemical imbalance in their system as you list. There are a lot of reasons.

I liked the Executioner book I read, the first one. Remo Williams is meh; its really up and down in quality from book to book. I recommend Chuck Dixon's Levon Cade series for just plain violent fun.

after a certain amount of processed sugar (in my case it was at around 29 years old lol) intake, that's everyone pretty much. we eat incredible amounts of processed sugar and eventually - sooner or later - we all end up with metabolic disease caused by it

I'm not buying this at all. Sloth and gluttony. Eating and drinking too many calories (and not good calories at that) and sitting on your ass all day will make you fat, and this is why the vast majority of fat people are fat. I've been everwhere from 130 to 215 over the last 30 years. When I eat reasonably and exercise regularly, I'm more toward the former. When I don't, I put on fat like Carni Wilson at Golden Corral's chocolate fountain. People want to complicate this issue, because it lets them off the hook and excuses them from the discipline and hard work necessary to be in reasonable shape.

I had a friend in HS who was lovely, petite, small boned and very pretty. she also had a birth defect (club foot) that had been surgically repaired but required repeated surgeries. she did SOMETHING to her foot during high school and it fractured. no more moving around, very little activity

she started gaining and with her weight growing her foot had more to do. it got worse.

slowly over years just worse and worse

your weight goes up and you don't feel great and your legs/feet can't handle it and your cardiovascular system can't handle it so you take it easy and then....

32 I got a device that pumps selzer into water, and found these great packets that give you citrus flavoring for your water with no sugar, no ANYTHING except crystalized citrus (lime, lemon, or orange). We drink nothing but ice and selzer water with flavoring around here.

The initial outlay for the device was over 100 but we were going through that in cans of fizzy water every month. The cans of C02 are about 12 bucks refill each, and each can fills 60 liters of water so they last quite a while. Totally worth it.

I think I heard about Ozempic being used off-label for weight loss maybe a year or two ago. Then I started hearing about shortages of it right after. Americans love a good weight-loss scam.

From what I hear, at least, once your body gets hooked on it, you're stuck. You have to slowly titrate it for a long time, or you'll blow right up again immediately, and whether or not you do, most people don't have the willpower to live on salad greens forever without the drug.

It really looks like a way to put you on the hook. If being thin is worth that, then by all means, do it. But the price in this case seems to be that you have to use it forever to maintain dem gainzzzz.

My brother lost about a pound a day just by not drinking coca cola any more. You can target stuff like that, don't sub it, just cut it out. That's a lot easier than "no sweets at all!" just pick one bad thing you have routinely and just stop that one thing. Then, after a couple months, maybe another. Its slow, but gives your body time to acclimate to the change and adjust to your new diet and lifestyle.

After a year of this you'll be amazed what has happened.

58 Not being 29 anymore, I suddenly have a middle that I never used to have. Sugar and the carbs that comes with it are my nemesis. I ordered a book recommended to me from Amazon '21 Day Sugar Detox: Bust Sugar and Carbs Cravings Naturally. Friends and some healthcare staff have done this and have lost a lot of weight and feel and look better. Easy? No. Still need to exercise control especially the first 14 days and keep moving.

I read the beginning of the article about the girl who was allegedly being fat -shamed. Maybe it's not always about you, intersectional victim number 43285...maybe the trainer, in the course of her long career, normally deals with patrons who want to slim down, even if they're a size 6 as opposed to size 16.

Yes, patrons join a gym to try out new routines, but don't act like you're gonna need reparations because a gym trainer assumed you wanted to get some tips on how to slim down.

That was the case with my father in law… a year before he died he started saying that his food either “ did not taste right”, or it did not have any taste to it. He became really i different about eating..

In this case, I'm pretty sure it's unintended. I remember the drug being around for several years, and it was intended to treat an already lifelong medical condition (diabetes), so there was no "cure" to peddle. The user was always assumed to be locked-in, unless something better comes along.

IIRC, the off-label use was originally a Hollywood thing. Models and actresses looking to step up from cocktail waitresses to "third drunk lady at the hen party in 'Summer Grossout Teen Sex Comedy XIV'" figured out you could drop pounds on the double with it.

110 When I used to go to the gym faithfully, it was the usual crowd of college-age to around early 50s who frequented the place. Everyone was reasonably fit.

Then one night a very overweight young woman showed up and started working out on the stairmaster. She came in around 9pm (the gym closed at 10), Thereafter I'd see her arrive late and begin her workout just as I was leaving.

I changed my workout timing, so I didn't see her for about six months. One evening I showed up late again, and there was the fat girl, and there she was on the Stairmaster...at least 50 pounds lighter. Who knows, maybe it was a hundred pounds she lost, she was considerably thinner. I always admired her for her discipline and the results she got. A similar situation occurred at a hiking trail I used to frequent.
There was a fat girl who was there all the time, and little by little, she shrunk down to healthy size.
And I never heard either of them cry about fat-shaming. Kudos to them and anyone else who has the determination and discipline to do what it takes to slim down and get healthy.

112 Fat people are not fat because they're particularly lazy. They're fat because their bodies don't properly regulate their blood sugar. Their bodies would probably be pretty healthy during periods of food scarcity, but, of course, the western world doesn't see much food scarcity. It sees mostly food overabundance.

I don't buy into that.

My oldest friend is obese. He even got band surgery and lost a ton of weight, however -

he has lousy eating habits and they didn't change. Yeah, yeah. He'd get the cup of soup not the bowl but it would always be some big calorie pit like Clam Chowder. Yeah, yeah. He'd get plain ice tea but then he'd dump 5 or 6 packets of sugar in it. Things like that.

And then combined with no exercise...roughly two years later he was worse off than before the surgery.

Bad habits combined with the nature of fat cells, which never go away. They merely shrink. So, they're always ready to load up on fat again.

This drug may be a godsend.

Weight control drugs aren't going to keep weight off. They may help you LOSE it, temporarily, but if you don't change your actual eating habits that got you there in the first place, then as soon as you stop the drug it piles right back on.

Weight control drugs are just a substitution for fat people wanting to lose weight but not actually wanting to put down the fork.

I was pretty chunky for a while and I finally made the actual decision to lose weight recently, and so far I've lost 12 pounds in a month (want to do another 20) simply by not eating junk, drinking water instead of soda, and eating reasonable portions of meals.

116 I commiserate with that woman for one thing: I find gyms mind-numbingly boring. My doctor said that because I've been on calorie-restrictive diets since I was a pre-teen, cutting calories won't work for long. I have to exercise a LOT more to keep my weight down.

I'm on an exercise quest. So far, I'm not crazy about yoga.

The slim women on my maternal side, including my mother, were poor role models. They ate only once a day, with beer supplements on weekends. (And cigarettes for many years.) The only exercises Mom taught me were arm circles, leg-lifts, and something called a "waist-whittler".

Based on ace's enthusiam for the series, I bought a collection of Ian Fleming's Bond books. I'm a little over halfway through the first (Casino Royale). Fleming is a pretty good stylist and it's interesting. I am now dumb enough to think I could play Baccarat.

Still working out. Still making progress. I am dinged up as hell though. Sore knee from exercise bike. Sore elbow from lifting. Can't tell whether I'm dealing with mild injuries or arthritis. Going to just try and work through it unless the conditions worsen.

130 I'm starting to enjoy viewing these images of young obese females flaunting their blubber, because it gets me to wondering what percentile I"m in with respect to being in decent shape, as in looking good in a bathing suit.

I'd say that I'm at least in the top 50% of all females wrt how I look in bathing suit, but in my age cohort, maybe top 20%, I don't know. How does one gauge this metric?

As the fat positivity movement took hold, I'd look around and marvel how out of shape so many young women were, then I realized, dag, their fatness is making me as an old lady look better.

I think it's in a similar vein as old folks losing the ability to taste food and losing the craving to eat. Lick an ashtray and then tell me how hungry you are. Yeah, don't, but you get the idea.

A tongue coated with nicotine is gonna block the taste receptor feedback mechanism. It simply doesn't taste so good and you eat less.

I think there is fat-shaming, but not so much at gyms. There are a lot of assholes out there who get a kick out of picking on people.

Several years ago, a 12-year-old girl moved into our area. She was very fat, but she was determined to keep up with her new friends on her bike. Some drivers - not just teenaged boys, but adults - yelled insults and one even threw a waterbottle at her.

Fortunately, normal people outnumber assholes and she ended up losing weight and making a lot of good friends.

I really used to enjoy working out on the rowing machine. But over time it seemed like gyms started getting rid of that piece. Then a gym popped up that was rowing machines and bikes only but it was too far from my house.

With the new immunotherapies for cancer, normally they will cause folks to gain weight (since you usually also need steroids, and starving oneself while killing all your cells is a really bad idea). And if you're a certain type of blood cancer patient, you are not even allowed to attempt to lose treatment weight gain (with some treatments being forever) until 6 months post treatment.

So, some people will be heavy, and not their fault. I told my spouse when I start treatment, I'll be one of the only cancer types who doesn't waste away...which I guess is a good thing...sorta...

So, knowing I'll probably be in a glass house eventually, I try to help folks on this issue...

There's a show she likes called "My 600lb Life" where "Dr. Now" has developed a program to help a certain class of morbidly obese people (> 600lb). Before jumping straight to surgery, though, he guides them through the process of learning to make different lifestyle choices. Bear in mind these people sought this particular doctor out, and he tells everyone the same basic things, but 9 times out of 10, it seems, they decide his advice isn't right for their particular circumstances, ("he doesn't know what's right for me") so they decide to ditch his advice and go their own way...and that ends about how one would expect.

Like one year it was those soft round things you balance yourself on, then it was kettlebells, then it was those resistance bands. Last time there were these little items that I don't even know what they're called but they seem to be used for resistance and balance.

Those are proxies that anyone can measure with a tape and scale - but they are still proxies.

From what I understand, VO₂ Max is a greater measure than any other in terms of heart health - which is really what we are after. So instead of designing a program to "burn fat" or some other arbitrary goal, it would be better to get insulin resistance where it should be, to get enough of the right kind of exercise to boost VO₂ which is going to require for many at least a pattern of vigorous walking.

The weight and other vanity proxies will follow.

In the '90s, there came this thing called Low-fat/High-carb Diet. I was taking a medication with a side-effect that I'd gain weight, so I went to a dietician. She all but called me a liar to my face when I showed her my food & exercise journal as I started gaining weight on the 1200-calorie plan.

Some of my co-workers were participating in the Weight Watchers at Work program, which reduced fat grams but increased fiber, including breads. A couple had similar problems because they gained weight if they followed the program.

I really like my deskcycle. Gentle, low/no impact exercise, with variable resistance settings, and you can do it while sitting down and doing something else like working on the computer.

It keeps my legs from getting stiff and achy from sitting at my desk all day.

The only downside is that you can work the joints in a chair apart from the cycling motion. And the potential creaking of your chair might tend to annoy everyone around you.

There may be some truth in this, but you can not discount the effect of the nicotine on the body and appetite (separate from the taste.)
Also, all the smokers I used to know would skip dessert and snacks and go straight to the puff puff puff. Often outside in the cold (another weight loss problem is our climate controlled life.)

289 Ozempic helps some with hunger, and some with sugar control. It isn't a miracle drug, it just makes it easier to eat less (especially into calorie / energy deficit).

The thing is, I know people taking it who act like it is a get out of jail free card. I don't think it will make a long-lasting difference for them. People using it to 'fix' their eating (particularly if they have anxiety when they get hungry), and really make better food choices (making this more like a cast on a broken limb: a temporary support while healing) are much more likely to succeed long term. I wish there was more real mental help to go along with the drug exuberance.

290 That fitness article has to be lying by omission. In 2016, that woman certainly was NOWHERE in the shape she is now in the photographs. You don't do 80 miles on a bike in a day regularly and look like that unless your diet is complete shambles.

Some will say that you just need willpower and intellect to get yourself out of obesity, calories in, calories out etc. That is unfortunately not reality. It’s more bullshit.

So, you have to try more advanced measures if you are truly “obese” or have eating disorders etc. That’s what the drugs and surgeries are for, rightly so.

Count yourself lucky if all you have to do is get a meal plan and do light exercising to lose weight. That’s not what we’re talking about re a large swath of obese people.

Many MMA fighters did it, because regulating your blood sugar is a quick way to really boost your testosterone without looking like there's anything fishy going on, you ferry caught you just go "oh no i should see if i have diabetes"

These drug companies saw this and are now just finding creative ways (1,000s of planned stories and celebrity endorsements) of advertising off label use of their products, which i think would be illegal if they were caught.

298 Diabetic here just started on Semaglutide - blood sugars improved slightly, dropped about 20% on average. Some weight loss, definitely suppressed appetite and I get the shits every few days.

It makes it easier to not eat, meals are smaller sometimes. I can feel starting to get into a situation where you eat less, so you make it count. Say you have a sandwich, rather than a lot of bread, less bread and more flavourful fillings. Same approach across different meals.

TBH, I'm a bad diabetic at times, I've done Keto and lost the initial weight, even when doing gym, but thats where it ends. It sucks just eating keto without the weight loss. Brother lost about 40lbs on Keto, sticks to it and looks great.

I think if I went back to gym and did keto, on these tablets I would lose it quick. I plan on getting back to that this summer.

Since 1/6/23, I've taken my weight down from 233 (obese according to the BMI Nazis) to 203 on 4/2/23 (merely overweight) and gotten off my hypertension and hyperlipidemia meds which was the main goal.

The doc says my blood fats are healthier now after getting off the meds and losing weight than they were.

How? Fasting every other day and eating Atkins-style on the other days. And weighing myself daily to keep on track. And giving up wine for Lent.

Today, I weighed in at 202. I'd like to get another 10-15 pounds off, but it's been nice to take a break. Giving up my reds was the hardest part.
