Weird News Dump

rise-shine-20160921-118-768x947.jpg

Another Saturday. Another day closer to Christmas. Do you have shopping on your Saturday agenda? If not, check out some stories that didn't get too much attention here at the AoSHQ this past week.

And remember, if you don't want to leave the house you can purchase The Deplorable Gourmet cook book. Honestly, there is more to the book than bacon and Valu-rite recipes. Not many.

*****

Enjoy these or other stories in this open thread.

***

Well Morons. That's enough weird news for the morning. Feel free to discuss these or something else in this open thread.

Remember to play nice with one another, or you could end up in the next Ace of Spades How To Book. Just sayin',

Have yourself a wonderful weekend!

1 Dear people of Alabama, you know what they say about the Sith.

There are always two, a Master and an Apprentice.

The Master, Trump, is literally worse than Vader.

The Apprentice, Moore, wields his oppressive religion like a lightsaber of death. He is also rumored to have gone after younglings (IYKWIM).

So on election night, vote for a real Jedi Knight. Let Doug Jones be a Force for good for your state and for the nation as a whole.

*Yoda voiceover*

"Fear leads to anger, anger leads to hate, hate leads to suffering."

Don't let Liberty die to thunderous applause!

That's even funnier when you realize the suicidal Muslim angle of the original trilogy, and the murderous peacefulness of the Jedis in the prequels.

The Jedi wouldn't be campaigning against Moore. They'd already be trying to throw him out a window 100 stories up.

According to Twitter CNN has already said they wouldn't punish the author as he followed protocol. Which tells us something about their idea of protocol I guess. Apparently, having two sources hand you information is the same as verifying the information twice, at least in their world.

What has happened when *Glen Greenwald* is the straight-arrow calling the organization to account and explaining to lay-people all the ways CNN screwed up?

85 Just opened the mail and found the new bill for health insurance for the coming year.

Michigan Blue Cross/Blue Shield, premier PPO silver plan for two people in good health - $2564.69 per month.

This is not going to happen. I will not pay over $30,000 a year for insurance that has a $2000 deductable per person and $7350 out of pocket (per person) max. On top of that, the plan only pays 70% of the cost.

Every doctor I know in their late 50s are looking for their exit strategy. I'm hoping to move my retirement date up by about 3 years.

If you consider that a typical physician's productive career (after residency) is perhaps 30 years, shortening that by 3 years amounts to as much as a 10% reduction in the experience physician workforce. You will never hear about this in the so-called news. But believe me, it is happening. There has been a surge in physician extenders, but rather than extending physician capabilities, they are filling what used to be doctor slots. Drop off in quality will happen gradually enough that most won't be aware of it. But it is real.

.We had to cave in and buy civilian Blue Cross/Shield health insurance through the wife's work (VA) so we can at least get something other than the VA for our health care as we can't find anyone who will accept our Tricare Navy retired insurance within 50 miles of us.

The only place we found who would take it was in Plano, TX about 55 miles south of us any other place who might take it would be out of network and we can't afford to pay those non covered amounts. And I wouldn't even consider that a real option on having to drive into/out of Dallas traffic for an hour to see a doctor is a non starter for me.

I hate to admit this but I use a United Healthcare Medicare supplemental plan, the really cheap in-good-health one that's like $35/month, and it's been great as far as minimum out of pocket charges and receiving excellent care. It's wise to seek out a doctor experienced with the ins and outs of Medicare as far as making sure your charges are minimal.

I don't know a lot about insurance. I was at a company that offered cheap coverage and downgraded every couple years to restricted policies. Not a problem in the metro area because with so many doctors in an area, one was bound to be in-network.

I was rather shocked when I started working here that I couldn't opt out. My doctor is about an hour's drive and my dentist is farther than that.

Posted by: Tami at December 09, 2017 09:24 AM (Enq6K)

That doesn't sound right. Routine waiver of co-payments can constitute insurance fraud, but I've not heard of a non-par provider taking cash being fraud. There might be some weird state that does this, but I've not heard of it.

Posted by: Insomniac - Nobody in Particular at December 09, 2017 09:49 AM (ywNoU)

169 My understanding is that it would only be fraudulent if the doctor's office accepted a cash payment from you and then submitted to the carrier for insurance reimbursement as well.

Some doctors will accept the insurance allowed amount and then negotiate the balance due payment from the patient if paid at the time. Unless it is specifically prohibited by the payer contract. I know Medicare/Medicaid prohibit billing the patient for any balance left on the total billed amount above the Medicare/Medicaid reimbursement allowed amount. This is why most practices restrict the number of Medicare/Medicaid patients in their practice.

182 I talked to a pediatrician yesterday who tells me he is routinely getting stiffed or low-balled on reimbursements for Medicaid patients in one of the managed care networks.

Here's how it (hypothetically, ahem) works.

The state will contract with a private insurance company to manage a pool of Medicaid patients and give them a lump sum for a given block of patients. The insurance company gets to keep what isn't spent, so is highly incentivized to keep expenditures down, sometimes at the expense of quality of care at the point of delivery. They actively discourage referrals to specialists and provide incentive payments to primary care doctors who stay below certain thresholds of referrals and test ordering. I think that used to be called a kickback. They also deny payment for some services, or pay so little that it doesn't cover the doc's overhead, or drag their feet on paying, hoping some docs give up in frustration at trying to get paid.

That is similar to what I proposed awhile ago. Amounts are for illustrative purposes only but are what I would set them at.

Every "US Person" gets a catastrophic plan at $10,000 deductible per year per family or $5,000 for an individual. This is paid for by the Federal Government which asks for bids from insurance companies. Everything after the deductible amount is covered.

Then also, as part of the same law, Health Savings Accounts are given a tax deduction up to the annual catastrophic deductible and any amount in the HSA that exceeds twice the catastrophic deductible can be taken out for other purposes tax free.

Subsidies provided, under tax law part of the same law, for those whose income is less than 10 times the catastrophic deductible, ($50,000 for individual - $100,000 for family) which would be a refundable tax credit to their HSA equal to the catastrophic deductible minus 10% of their income until their HSA equals the limit of twice their deductible.

Positive incentives to government by tying the catastrophic deductible to all other amounts. To insurers by competing for the huge catastrophic market and encouragement to lower large medical expenses while improving the care so people will choose the lower cost alternative. And to the people insured to spend wisely so that their HSA money can be used for other expenses.

This would replace Medicare, Medicaid, and other government healthcare programs, which right now run over 1 trillion per year, whose funding could be used to cover the costs. The problem of pre-existing conditions goes away since in the catastrophic policy they are priced into the bid to cover everyone and becomes an actuarially accounted for figure which would not vary much year to year over the 330 million people covered.

214 I think it's because of agreed upon rates for procedures. The $500 was knocked off by the insurance company because the remainder is the 'agreed upon rate' they have with docs. I think...maybe.

Right Tami probably. But shouldn't that be shown/accounted for on the statement.

215 I have a bill from my cardiologist - $998.00 in total charges. For what they did, that seems reasonable but - my Ins paid $460.85 which, according to the statement, leaves me a balance of $66.91.

Automagically almost $500 was applied or forgiven by something, someone to my balance - but it isn't explained anywhere.

Wut?

*********

Let's say f'rinstance:

Cardiologist list price for a given service is $1000

Cash customer negotiates a 30% reduction- pays $700 out of pocket.

Medicare allows the doctor perhaps $500. CMS pays doctor $500. Patient pays $0. Doctor 'writes off' or adjusts books $500.

Medicaid allows the doctor perhaps $300. CMS pays doctor $300. Patient pays $0. Doctor writes off $700.

In your scenario:
Private insurance in net has contract with doctor for that service, will pay up to let's say $600 with no balance due. Insurance pays, its percentage of that (let's say 80%) send doctor $480. Patient is responsible for the balance, pays $120. The doctor writes off the $400.

Doctors will set their list price higher than they expect to get paid by any third party payer, and will sometimes get paid the full amount by unwary cash customers, which is a bonus. They don't really expect that anyone will pay the full amount. Next year the doc may charge $1200 for the same procedure. The insurance and CMS will still pay the previously negotiated (or dictated) amount.

220 I left out the out-of-network scenario.

Doc bills $1000. Insurance pays out of net rate of say $500. Doc bills patient for the balance of $500. Might be willing to negotiate a discount, might not. Out of network patient is responsible for the full balance.

The in-network patient gets the advantage of the agreed-upon reimbursement amount.

There was no indication Obiwan was a clone, no.

There were a couple references to someone other than the clone troopers being cloned, but not that I recall of any jedi.

Also, there were a couple plotlines that touched on the jedi's insistence on recruiting very young (one of which included some handwaving about how they could find every child in the galaxy with force potential) which would make that unlikely.

I would say the fact that democrats are rapist hiding behind woke speech is no reason for conservatives to pay.
It is time for the democrats to be the whipping boys for their own crimes for a change.

I would also say that this is very typical that conservatives are always expected to pay for what democrats do. Taxes, Fees, Rights, Public Scorn, you name it, whatever a democrat does the conservative is expected to fork over the repayment, the penalty, or act as the designated goat so they don't have to.

"Medicare allows the doctor perhaps $500. CMS pays doctor $500. Patient pays $0. Doctor 'writes off' or adjusts books $500. "

CMS pays 80% of the $500 after you have met your annual deductible. So they would pay $400 and you would be responsible for $100 or more if you haven't met your deductible.

This 20% is what you get a "Medigap" policy to pay for. Some just pay the 20%, some will pay the deductibles too which really helps if you get hospitalized as those are separate deductibles of over $1000 per admission. But you have to watch what Medicare will cover because if they decide to not cover something then you will be responsible for all. And it is hard to make sure something is covered.

My wife went in for an annual checkup a few years ago which was supposed to be covered from dollar 1. Whatever the doctor did or sent to CMS as the procedure code was not covered and so we got billed several months later for over $300 - unexpectedly.

Medicare Advantage plans, which is what we now have, are more affordable and easier to figure out just what your out-of-pocket costs will be.

Still, that couldn't have been an easy thing to carry around. And whatever led him to suicide, that's always tragic.

Speaking of, how much of so-called journalism these days is paid adverts by this that or the other entity?

How many articles do you see in your local rag for a restaurant, some cultural or recreational facility, praising it to high heaven? Was money exchanged? And they're calling what they do "news?"

Yeah, that's a scandal I'd like to see revealed... but then somebody would have to do some actual, you know, journalism.

No question about that. If you're 21 and female, how bad could you possibly look that you have to mutilate your face?

Reminds me of an old old old National Lampoon article they printed... oh yes they did, called "My Vagina." It's terrible, but I first encountered it when I was a teen. About a boy who woke up one morning and found out... he had a vagina. The ensuing "adventures" were hilarious (to me, a teenage boy).

And somehow that stuck with me, to the point where I think, come on ladies, it can't be that bad. Any time you want, you can ring your own bells.
