I've been in some really old football stadiums. Anyone else ever seen the urinals that are like one long gigantic bath tube that everyone stands around the outside of and pees into?

Now, *they're* built for crowd control. Never a line for those suckers, you could probably fit 100 guys shoulder to shoulder around them.

But we do have a busted cast iron toilet flange on the second floor, and a tiled tub wall that is leaking water into the wall around the shower fixtures and down onto the top of the living room ceiling.

LOL Couple of years ago we took a gondola up the Alps near Innsbruck. Got out at the top and there was maybe 50 feet vertical left with a somewhat steep but easy trail to reach the very tip top. Took about 10 steps and nearly passed out. Husband said my lips were blue.

Very embarrassing sitting on the bench while the 90 year old natives who had actually hiked from the bottom passed me by quite spritely.

82 On my walk today there was section of a grassy area that looked like snow had fallen. It was thousands of those drifting white seeds from dandelions and the dew had made them wet.. It did sort of give me a sense of what manna in the wilderness might have looked like.

Truth be told, we're only at 8,500 ft so the altitude isn't affecting me. Back when I was 27 or 28 years old, hiking up some of the 14,000 ft peaks in CO would definitely be slow going the last little bit.

A few years back, early one spring after a particularly harsh winter with deep snows up around Gunnison we counted over 130 deer carcasses along a 35 mile stretch of two-lane highway. Apparently they just died of starvation because they couldn't find enough forage or move to reach a better area because of the deep snow. Come spring the snow receded, leaving the deer like so many beached dolphins left behind by an outgoing tide. It was incredible.

I just re-read Vic's comment. He referenced Medicaid AND Medicare- which have different reimbursement schedules. In my line of work I deal much more with Medicaid (rather used to- I am salaried now and the day-to-day billing is no longer my concern). Medicaid is somewhere in the range of 70 to 80% of my patient population and at current reimbursement rates I am not earning my salary, much less my overhead. (My position is subsidized in various ways).

There may be service lines where you can break even at Medicaid rates, but those are bare bones services (or as alluded to upthread, revolving door billing mills)

Medicare is different.

so the trick as EDU sez is to run a mill and double or triple book.

Had a talk with a gifted orthopedic surgeon serving in a rural area in Southern VA. He's thinking of retiring early primarily because the Electronic Medical Records (EMR) requirement is completely untenable for a small private practice, thus leaving four or five locals without jobs in his office and denying that rural area of really good orthopedic services.

As an aside, I always thought that EMT is a key step in establishing dispassionate death panels: "The computer has reviewed your records and given you the No Further Care - Thumbs Down status!"

I see your point, but there are ways to game the coding of medical services. Office based services are coded on a scale of complexity of medical decision making, with various criteria needed in your documentation (clinic note in the event of an audit). These criteria can be met by a time element (how much time you spent) or by a documentation element (how many and what type conditions did you evaluate, what tests did you order and interpret, what treatments did you describe, etc. ). From simplest to most complex this is a scale of 1 to 5. (99231, 99232, 99233, etc). Level 5 is roughly the equivalent of one hour, as you allude to.

I have one particular colleague who at times will see twenty patients in a six hour clinic day and bill every single one as a level 5 service). I find this dishonest at its heart, but as far as I know he has gotten away with it.

And you wouldn't think so due to his field but he has, a number of times, caught things that sent patients to a different type of specialist.

267 Muldoon the only hiking I've done in CO was in winter. One snow-shoe day around Rocky Mtn. NP, and two trips up from Aspen to the 10th Mtn Div hut up there above it (@ around 11K feet). The hut trips were mountaineering skis (first time), snow-shoes (second time, having learned the first time that skis made no sense for that climb).

We were the only people in the hut, which was brand new at the time (and built for 12 or more). Spectacular.

So true, and it is absolutely not isolated to the field of medicine. Whenever a powerful government intrudes so extensively into what should be an open market, any sense of proportion regarding pricing and compensation gets blown away. The market is replaced by an unending cycle of rent-seeking.

When you have a service provider who has a fixed notion in their mind of what their service is worth, and a payer who has a different fixed notion of what they are willing to pay, and rules that are imposed on both parties, everyone involved with try to manipulate that system to their own benefit.

I'll bet you could not find me a physician who thinks their worth is less than (oh, let's say for the sake of discussion) $150k. But in a market economy there are may who would make less than that. And maybe seek another line of work.

My Dad was posted to Italy (Vincenza). We live off post in a "villa". Lots of marble floors, center staircase, big fireplaces. (cold, very cold).

One was ordained with me and one happened to be the pastor of a church I attended. The last one is about 6'5. I'd have to find someone really close to God to find someone taller than that., I think The last one quoted to me, "I must decrease so he will increase" which is from John 3;30. Two of the least attractive men in my life ( a best friend, and partner of 10 years) were the best. They were good, decent, nice men who loved me and genuinely wanted the best for me. Almost like an unconditional love, very accepting.That kind of man (or person) is hard to find. Worth their weight in gold those two.

343 Well, last week we finally had all our hurricane damaged trees and "leaners" taken out. The back and side yard are a mess with ruts and the grass is destroyed.

So now we have to have a couple of truck loads of dirt hauled in and the yards re-sodded.

Just in time for the start of hurricane season June 1.

It's a blessing to find people like that. I attended a funeral service of a man yesterday and almost everyone spoke of his unconditional love for people. It was very touching because it was a large crowd at a church. Sometimes there are funerals where almost nobody shows up because the person was miserable and alienated everyone-even their family, so funerals where people are can genuinely celebrate a life are great.

I only wish I could visit my late dad's grave in FL, and his dad's grave in upstate SC and place flags.

There are many women who feel the same way as you. But I've noticed it's usually women who are comfortable with themselves--whether they are "average' or not. Women (and men) who are not comfortable with themselves or their looks often seek out a handsome (or beautiful) partner to boost their own self-worth or social-standing.

Arimatheans are caring parishioners who attend Funeral and Memorial Masses as representatives of the faith community.
Does your faith and church have such a group? I think it is the kindest thing.

352 Rats are great pets, especially if they are raised from pups. I had one who drank beer, chased my cats, and would come when called. The only problem with the little guys is that they don't live longer than a few years.
