We never actually used it, though we certainly put out word that we might. We just did. Possibly in retaliation for a Green Beret's recent death there. The bomb weights 21,000 pounds. The second video below shows the test detonation. It has a blast radius of a mile.

The opioid epidemic started due to pill mills in places like Florida handing out prescription painkillers for anyone complaining about a sore back.

Taking away pain relief from people that have no other options is evil. / I'm not talking about "back" problems this is pain caused by neuro conditions (nerve dysfunction). I've known people who had permanent, intractable, life-destroying pain which could be controlled only by opioids. No other instrumentality would suffice.
They took opioids not because they wanted a kick, not to have fun, but simply to lead something like a partially normal life not filled with constant appalling misery. They weren't too thrilled with the side effects, either, and wished every day for an alternative that worked better. There were none such. I was strengthened greatly in my native belief that medicine is NOT the proper purview of the state by watching their experiences and struggles. I am becoming furious with this nonsense about making it harder to get prescription pain relief. As for junkies, I frankly don't give a damn. More overdose deaths among "recreational" drug abusers don't budge my needle of compassion at all. I save mine for people who are physically broken and unrepairable. When I was in college here in S.F., I was on the debate team. One of our rivals was UC/Berkeley -- they were pretty much stuck-up geeks (nerds with no redeeming social qualities) except this one guy, Evan, who had long blond hair and kinda looked like a viking. I debated him 3 times and every time, he swore he won -- he didn't. Anyway, after I gradumacated I learned that he OD'd on heroin. I had no idea he even used it -- the drug of choice in the college debate world was mostly weed with some occasional acid. Not everyone who uses heroin is scum or a loser, just as not everyone who drinks alcohol is (and alcohol kills more people every year than all other recreational substances combined).
It's a sad situation. Thing about heroin addiction is that it is so physically hard to kick. After my dad had major back surgery and got hooked on morphine, he went cold turkey. My mom tied him to the bed for a week and we kids had to be very quiet. He was miserable beyond belief. I can understand why someone would commit a crime of opportunity to avoid it. About the opiates-
I was working for a family physician around 1999-2000. A drug rep came in and offered the doc an all expenses paid trip to see a Eric Clapton acoustic performance. Expensive hotel, best restaurants. On the way to the concert the drug rep pulled out about an 8-ball of hash. By the time we got to the restaurant...honestly I don't remember much from the restaurant. So after our stay I asked the doc wtf does that drug rep want you to sling? I'd been on a few of these trips before but this was extraordinary. He said that the drug rep wanted him to prescribe a drug that was originally developed to ease terminal cancer patients pain during their last days. It was so potent that it would cause addiction but...terminal patient. But the drug rep wanted him to start handing it out for people with broken bones and chronic pain. The doc said he thanked him for the trip but he wouldn't be handing out scripts for such a potent drug which he described as heroin in a pill. That's why Oxycontin and opiate abuse are such a plague now. Pharma sent reps into small towns around the country and got people addicted to their legal heroin. I will say that when I have kidney stones, only opiods touch the pain from that. Lesser drugs do nothing. There's a role for opiods, they work. But they shouldn't be used recreationally. So they still do derive benefit from treatment with opioids. They don't get 100 percent pain relief. They get 100-Y percent. It's still entirely worth it to them to get that number down somewhat. I have a problem with the "all pain must be eliminated" mentality, because I think it veers dangerously close to the same mentality used by the pro-euthenasia crowd.
Remember that assisted suicide was supposed to be for the terminally ill: cancer patients with a few months to live and the like. This whole restrict opioids thing doesn't make a lot of sense to me.
Most of the studies which I'm aware of show that prescribing opioids for pain say after surgery shows no addictive effects if the patient uses the drug only for as long as they need it to relieve pain Obviously. this doesn't include longterm use as in chronic pain, Or drug addicts. I'm guessing a big part of the problem is patients who decide they like the buzz using their leftovers and refills for recreational use. As a sample size of one, I've had a couple of surgeries where they loaded me up on opioids, which I took, then stopped taking when I didn't need them, aaaaaaand- nothing. I went on my merry way. Yeah, I'm not fully up on the science around it, but there appears to be evidence that if your pain level is X, and the painkiller you take is measured to reduce/eliminate the pain, your receptors will use the medicines you need, and essentially leave no "leftovers" for the addiction part of your brain to mess with.

Here in Delaware your primary care physician has just had his ability to prescribe opiate pain killers revoked as of April 1. "Pain management" clinics and their staffs are now the go to managers for this problem. Mrs. Krebs requires opiates (ain't debating this, kiddos) to function with neck (auto accident) and lower back (unknown) issues. The earliest she could get an appointment with a pain management clinic was early July, leaving her with an over two month long gap where she would be disabled and unable to function from pain. All this because a state-created "blue ribbon panel" recommended a change because there are abusers out there. Mrs. Krebs is not one. She heard from her PCP this week that he has leave to continue to prescribe opiates for her until she is on board with the new place in early July. Got a bone spur right behind my Achilles that's killing me as well as chronic knee pain from a few too many aggressive hiking excursions. Given up on anti-inflammatories and pain killers as all they do is take pain and inflammation that's a 10 and make them a 7 or 8 but with the side effect of screwing up my stomach and giving me deadly constipation. My only routine medication is L-thyroxine for hypothyroidism. I almost never take any medication for discomfort or pain. Mrs. Krebs knows immediately something is wrong with me whenever I ask where the OTC meds are. If I schedule a non-routine visit to our PCP, she really flips out. That leaves the pharma companies only 6 to 8 years to make up the cost of development, which now average over $2.5bn per compound. Then they have to also pay for the research that went into 9 other failed compounds that never made it to market. You don't want the pharma companies to advertise? Fine. I get it. But if you keep putting roadblocks in the way of pharmas making back their research and development costs then you're going to get less medicines and of poorer quality. The generics will not have much to copy at that point. Note that as of November 2016, the International Council on Harmonization has released new "guidelines" that basically add a whole new level of requirements on top of the already voluminous guidelines and regulations governing drug development so the costs aren't going down anytime soon - and believe me, we're desperately trying to do so. I'd love for you to meet some of the people I've known who were, regardless of how many opioids they took, in continuing pain every waking and sleeping hour. (And the loss of sleep makes it still more devilish. Fatigue slowly wears you down to a nub.) They'd have been praying every damn day for an opioid that would eliminate all pain. No such thing appears to exist. What they got by taking opioid painkillers was a reduction in pain to where they could somewhat manage to see their day through. Insomniac, I do have sympathy for some of those who get hooked, because a LOT of them are just stupid kids. They use them, their friends use them, and they DIE. My daughter who is now 26 has seen a number of her friends from high school days (a high school in a nice, suburban neighborhood) die from this.
Way, WAY different from the death rate of my high school friends. I inagine most of them think it's not much more dangerous than smoking grass, and if course *they* aren't going to get hooked. But man, it's easy to get hooked on opioids. (Which is my favorite drug, by the way, for after a surgery or dental surgery.) So I hear of one after another of these kids dying - and the nerve-wracking tales of ones hooked now and flirting with death every day - and it is truly awful. And awful for their parents. I can definitely see how you can get hooked on pain killers. I broke my collar bone and the docs threw a bottle of oxycontin at me. I fell in love with the stuff and quickly decided I either needed to regulate myself or make some bad choices. So I limited myself to a half-dose twice a day instead of a full dose 2 to 3 times a day. It was enough to take the edge off of the pain. I was kind of sad when I got down to my last pill. That's when I knew I should probably never try meth or any of the sexy stuff kids are into these days...I value gainful employment...and my teeth.

I'm a no drug type guy. It was real pain, but there was no reason for it. So I adopted a different approach than babying myself. The vast, vast majority of "recreational" drug users (to include dumb young kids) who overdose on opioids do so because their dose isn't correctly calibrated. It's not that they're intending to die. Just letting them openly purchase the stuff in a controlled, monitored way, where they know exactly what they're getting and how large a dose it is, provably does slash OD death rates. The UK did it just that way for a number of years, in one of the few sensible medical measures that country has come up with in the modern era. My first assignment was a medical hold unit managing soldiers with complex medical care. We saw everything from the guy who was blown up by an IED and had his skull rebuilt to the soldier run over by a humvee whose legs where shattered to TBI, PTSD, all the way to the jackass who fucked up his back jumping out of a deuce and a half. I'd say 90% of the "chronic pain" sufferers were full of shit. They wanted pain meds and the doctors were obligated to proscribe them. The barracks were awash in pills. Most of the pain could have been dealt with by lifestyle changes and physical therapy, but they'd rather get high and spend their day playing xbox rather than do their exercises and go on a diet. Ironically, the ones worst off were the least problematic. They'd do what was required to minimize their pain without drugs, and take the minimum dose necessary to function.

Why an anticonvusive for neuropathic pain? Well because its a different beast compared to normal musculoskeletal pain. It arises from damaged or destroyed nerves, not stimulus of pain receptors. To put it in layman's terms, you have to calm down the nervous system so its not reacting to the nerve damage. Thats why you use Gabapentin (which really is no longer used as an antiepileptic all that often) or Lyrica. Thats also why some antidepressants are effective, they have similar effects on how the brain processes those signals from damaged nerves. To be clear, this is seperate from their antidepressant effects. Opiods are usually not a frst line option for neuropathic pain.

Not sure what effect a 21,000 lb high explosive air burst would have on tunnels and bunkers below. Forces might collapse them if not to deep, possibly. Might consume enough oxygen above ground to suck the air out of them. Probably would make one heck of a crater in any case.

I know all about these nervous system disorders because ten months ago I suddenly developed a headache that never went away. For TEN MONTHS. Not your garden variety, take a few aspirin and it'll be gone soon. Stabbing with an icepick in the back of your head, headache with a throbbing, pulsing chaser on top. To the point that they have looked for brain tumors among other things. Just because pain is considered neurological in some diseases does not mean that it is "phantom". By the way, I HATE pain pills and normally would only take about 10 - 20 Aleves over the course of a year. I take gabapentin for nerve pain that began at the same time the headaches did. It works. I don't care why, I'm just so grateful that it does. Three MRI's, 2 x-rays, a carotid artery scan, about twelve different blood tests, and we still have not found a diagnosis. Days when you can't stand and walk or even sit fully erect. But guess what? I still hold a full-time job. So, fuck you very much. The one thing to remember about the GBU-43/B MOAB is that it is not in any way a hard-target penetrator like the the GBU-28 series. It's just a big thin-skinned can of explosives. Eleven tons TNT equivalent from what I can find. That'll rattle some hard-rock tunnels and a good hit would close the entrance, but these, just like the previous BLU-82, are aimed at heads, not materiel or facilities.
