Democratic Senator Tammy Baldwin Was Alerted to WI VA Scandal Months Ago, But Did Nothing

And now that the scandal has broken wide without her assistance, she's calling for an investigation, chop-chop, on the snap and stat.

But why did she not call for this investigation when a whistleblower risked his ass to tell her about it?

Tammy's still not talking.

Even as Sen. Tammy Baldwin calls for the U.S. Department of Justice to launch "an immediate" investigation into allegations the Tomah Veterans Affairs Medical Center in Tomah overprescribed opiate-based pain medication, the Madison Democrat has yet to disclose details on the reported firing of one of her top state aides in the wake of a growing scandal.

Baldwin did not return Wisconsin Reporter's calls and emails Tuesday seeking comment nearly a month after she reportedly fired Marquette Baylor, who led Baldwin's Milwaukee office.

An investigative story last month found Baldwin did nothing with an inspection report warning Tomah VA medical center staff members have prescribed potentially dangerous amounts of pain pills to patients.

...

But Baldwin did not call for an investigation until after an investigative report detailing the concerns broke in early January, months after a whistleblower reportedly begged Baldwin's office to do something.

Baldwin has now fired Baylor. I guess the Alibi is being set up: The head honcho didn't know, it was all this underling's fault...

9 Not to excuse Baldwin, but the entire VA Medical system is a scandal.

Its layers of incompetence, piled on top of waste, piled on top of more incompetence.

The best thing we can do is abolish the entire thing and give vouchers to our servicemen for private healthcare.

20 I'm actually not to keen on the Feds cracking down on painkillers so much.

I had a relative in a Medicare facility, recovering from surgery. It's a depressing place, all these old people in so much pain. Literally crying out in agony -- begging for pain meds.

But the restrictions are distribution of pain meds err on the side of prescribing too little -- cause docs can lose their license. For the individual docs and nurses and admins -- better some old lady writhes in agony then they get on the Feds radar for over-prescribing. It's a seriously messed up situation.

I don't really care if some 87 year old lady spends the last few years of her live with an opiate addiction if the choice is between that and being in agonizing daily pain.

The priority should be the pain management of the old, sick, and injured.

Will that lead to some extra addicts? Probably. That's a fair trade-off.

I know nothing of the players nor the scandal, but this bit is interesting:

Sources last month told the newspaper Baldwin's office offered a severance package to Baylor, a cash payout with a confidentiality deal.
Among myriad questions surrounding the reports is this: Would taxpayer cash be included in what critics of the secret agreement describe as hush money?

107 Wisconsin. I never thought much about the place. It always seemed boring. But it seems its politics are as weird as they get. They have a governor who resembles Mr. Rodgers and who has built a throne of skulls.*
They have a lesbian senator who has tried to cover up a VA scandal involving painkillers.
They have teacher unions that could show the ILWU a few things.

I think it may be the cheese.

It's a little bit more complicated than that.

There are definitely SOME doctors who do or have "sold" scripts for oxy or other strong narcotics. Those are pretty clear cut cases.

But now the DEA (DEA!) is going after doctors in less clear cut cases. Like, for example, a pain management doctor who is in the top 10 oxy prescribers in the state. As you can imagine, pain management doctors hand out a lot of pain pills. The DEA says "Hey Dr. Smith, we think you've written 1 too many scripts this year and you're going to jail" and Dr Smith says, "what do you mean?! All of my patients have been referred to me because they have chronic pain!" and the DEA says "well some may be faking it" and Dr Smith then has to prove that none of his patients are faking it (very hard to prove) and that every script is legit and up to the DEAs arbitrary standards, or he loses his license and possibly ends up in jail.

My mouth started to shout "but I'm really really really in pain and I need need need them", but my brain stepped in and said "that's why they're not giving them to you".

Posted by: Costanza Defense at February 18, 2015 04:13 PM (ZPrif)

Well, the problem with that is that there are people with legit back problems, and its hard to disprove, so a lot of scammers and addicts fake those injuries.

But that's unfair to the guy who really is injured, no?

And a lot of guys end up being prescribed opiates, then cut off by their doctor, and whether due to the pain or addiction or whatever, the guys end up going to street opiates which are easier and cheaper to get then a lot of these pharma pills.

I don't know what the answer here is, but I'm pretty sure I'd lean on the side of "if the guy cant work due to pain, give him some pills. Hopefully he's not faking it, but if it turns out he is, don't throw the doc in jail"

=======

I agree with you that in many situations patients who can benefit from narcotics aren't getting them. In general there is a lot of room for improvement in how narcotics are used. But the choice you present oversimplifies things a little.

Old people are much more subject to all sorts of side effects from narcotics. That includes constipation (which is no joke in older frail patients. . .it can cause serious problems), to low blood pressure, and even obtundation. Addiction is still an issue, even in older patients.

So is illegal diversion of narcotics away from older patients. For obvious reasons, old people may not quite be on the ball about what's going on with their meds making it easier for family and even healthcare workers to illegally divert them. .

The point is there are reasons other than simple paperwork hassle why physicians tend to shy away from narcotic use.

263 #215 Narcotics are really good for relief of severe acute pain.

If you had second degree burns over a good part of your body, or suffered some sort of other serious traumatic injury, narcotics would be good. You can't really become addicted to them, if they're administered correctly in that sort of setting.

Oversimplifying, the problem is when people start to use them in chronic settings, particularly when they have total control over administration. If you take narcotics every time you have pain, and the source of your pain is some sort of thing that isn't going to go away, well, pretty soon you're taking narcotics all the time, you're physiologically addicted, and you're about a half-step from being psychologically addicted.

And if you deliberately take narcotics with the intention of getting "high". . .forget it. Do that even a couple of times, and you're probably going to become an addict (if you aren't one already).

So. I worked for a firm where pretty everyone but me was Catholic. Nearly all of them were fairly devout too, not just Christmas and Easter Catholics. I mention this only to set the scene for just how humiliating this would have been.

One day, one of my bosses walks in and I look up and I'm about to say, in front of a bunch of other co-workers, "Hey, you've got something on your forehead" when, seriously, God slapped his hand over my mouth and was all NO. NO YOU SHUT YOUR MOUTH.

It took me noticing a few more people with dirt on their foreheads, followed by a discussion of what people were giving up for Lent, followed by someone saying "Aren't you going to Mass to get ashes?" to someone else before that penny dropped.

I think the Koran is a little vague on the subject, but judging by Muslims in the middle east and those that have come to the US or Europe... No, they don't feel they should have to work.

You have 3 or 4 wives at home to take care of the kids, and you hang out all day at the cafe drinking tea with your buddies and bitching about the Jews. That seems to be the Arab way of life.

Other parts of the Muslim world aren't so fortunate as to have a fuckton of oil wherein they can support a whole country sitting around doing nothing but having kids and bitching about jews.

But even Egypt needs huge cash infusions from their Gulf Arab buddies and us to feed their people. Nobody works!

Anyway, they actually have a Healthcare Ethics Division. There was an internship, I asked a simple question "So...does this internship require relocation to Seattle or DC?" (Which amazingly are the only two places they have offices, no satellite offices are listed.)

Never heard back. They can't even answer a simple question. Yet we pay someone SES money to head up that division, and probably a bunch of GS15s to do the work.

For what? Their own fucking website says they have no QI/QA powers (despite part of their job being coming up with those criteria.)

Bonus round: the head of the VA ethics office has a trademarked ethics program, I bet they pay her money to license it too.

Specialties tend to be very good, with the exception of Surgical. Almost all VA hospitals have post-surgical infection rates well above the norm.

Lousy oncology.

Excellent mental health care (and I say this as a total skeptic until about - oh....just after I got back from Iraq I would say), cutting edge prosthetics and rehabilitative care, good community based services.

VA isn't a total loss, but the mid to lower level bureaucracy really fuck things up for the actual care givers. They are why the VA sucks.

Tomah VA medical center staff members have prescribed potentially dangerous amounts of pain pills to patients

What if the overage was never actually given to the patients, but simply smuggled out of the facility and sold on the open market, and that the number of pain pills given to the patients were an appropriate amount and strength?

Would there be such market? for opiate based pain killers? hmmm...I'll have to get back to you on that.

For Ash Wednesday, Ash Wednesday:

http://bit.ly/1gbfu9m

One of my favorite T.S. Eliot poems. I always remember to re-read it on this day.

This is an excerpt:

Blessed sister, holy mother, spirit of the fountain, spirit of the garden,
Suffer us not to mock ourselves with falsehood
Teach us to care and not to care
Teach us to sit still
Even among these rocks,
Our peace in His will
And even among these rocks
Sister, mother
And spirit of the river, spirit of the sea,
Suffer me not to be separated
And let my cry come unto Thee.

I am not a man of faith, but I find so very much to contemplate in this poem.

The VA here is also hampered by the fact that it may as well be located in a gang war zone.

That cannot help PTSD patients with the occasional shooting happening literally right outside the parking lot.

I can't speak to clinical care, but I know research oversight was lax enough that they let the guy in charge of the animal program basically do the equivalent of "Defaulting" on regulatory compliance.

Now you may say "who cares it's government regs." Which is partially true, lots of NIH money pass through there, and then tend to get very pissy about enforcement of their regulations. IIRC the whole damned facility was shut down (and all the animals transferred to our university which was in much better shape.)

This community involvement seems a step toward sharia zones. He's proposing that the first line of inuiry when it comes to a radicalized youth is to have community leaders step in instead of police (if I've understood it correctly). Which seems silly when you consider that it is the community leaders - the mosques, the radical imams such as seen in Boston w/the Tsarnaevs, that are doing the recruiting.

How recent is his experience?

Because...stipulated....that as recently as 15 years ago it still royally sucked across the board.

But the metric fuckton of money dumped in there since 2005 has begun to have an effect.

It also matters greatly where you are. Care in Palo Alto, CA? Excellent.

Care in Pine Bluff, AR....ummmmm...not so hot.

But is Obama suggesting we set up a separate but equal community-based "policing" of yutes for Muslims?

Also, why do we care if they go to join ISIS? Let them go. Just try them for treason upon their return.

I'm not comfortable with thought police arresting people for posting the wrong thing on twitter.

349 This community involvement seems a step toward sharia zones. 315 Uh oh. A Mohammedan is sending out Valentine's (a Christian holiday, celebrating St. Valentine) cards to the SCOAMF.

There are some who believe that the Feast of St. Valentine was meant to co-op the Roman pagan feast of Lupercalia, which among other things involved running around nekkid, and "striking those they meet with shaggy thongs" according to Plutarch. The striking of the thongs was thought to ease a pregnant woman's delivery, or for those seeking to get knocked up an easier time of getting that way.

I thought people were being silly by calling him a Muslim in '08. I figured he was just a socialist / atheist / "multiculturalist" / "spiritualist" who pretended to be Christian when it suited him (in Chicago with rev wright, for example).

Now... I mean, I still lean in that direction. I don't think he faces Mecca and prays 5 times a day.

But I'm REALLY starting to think that he sympathizes with and prefers Islam over Christianity.

Like, religiously I'm basically an atheist or agnostic or whatever you want to call me. Don't think about god, dont go to church, don't pray. But if someone put a gun to my head and said "what religion are you most closely aligned with?" I'd say Catholicism.

I'm really starting to feel that Obama would say "Islam". Plus, all Muslims think he's one of them since his dad was Muslim, which makes Obama a Muslim. And if he really were Christian, you'd think there would be fatwas out against him for being a kaffir and leaving the faith, which is punishable by death.

Exactly! Community leaders do not = police inquiry. It's ceded control to unaccountable community leaders. And will they be tracking any of this activity, or held accountable? So that is lil Joker Tsarnaev, who was referred to community leaders twice over the last few months, is flagged by FBI - will the FBI even know that Joker's already been noted as a threat by locals, or will it be a case of pre-9/11 non-communication amongst agencies caused intelligence failures?

These community leaders don't work for the city/state/federal government and their priorities are not necessarily protecting the safety of the greater community (the city, America) vs. protecting *their* community.
