The era of big government is... continuing.

President Trump on Thursday rolled out what he's calling the Great Healthcare Plan, a sweeping proposal aimed at doing what Washington has failed to accomplish for decades: driving down healthcare costs by breaking the grip of insurers, drug companies, and middlemen while putting patients back in control.
"I am thrilled to announce my plan to lower healthcare prices for all Americans and truly make healthcare affordable again," Trump said from the White House. "We're doing things that nobody's ever been able to do."
At the center of the proposal is a call for Congress to codify Trump's Most-Favored-Nation drug pricing policy, which would require Americans to pay no more for prescription drugs than the lowest prices paid by other developed countries. Trump said the policy would lock in massive discounts already being achieved through voluntary negotiations with HHS and CMS, while grandfathering in those existing deals.
He argued the policy would push drug prices down "80, 90% in some cases," adding that some medications would see reductions of "300, 400, even 500%," figures he said Americans have "never heard of before."

He also proposed some small-bore changes to Obamacare.

A major structural shift in the Great Healthcare Plan involves insurance subsidies. Instead of continuing to route billions of taxpayer dollars through insurance companies, Trump is proposing that eligible Americans receive that money directly, allowing them to purchase the health insurance of their choice. "The government is going to pay the money directly to you," Trump said. "The big insurance companies lose and the people of our country win." The plan would also fully fund the Cost Sharing Reduction program, which the administration says would save taxpayers at least $36 billion and reduce premiums on the most common Obamacare plans by more than 10%, according to Congressional Budget Office estimates.
Trump did not shy away from attacking Obamacare itself, arguing it was "designed to make insurance companies rich." He pointed to billions in subsidies and what he described as skyrocketing stock prices for insurers while premiums climbed year after year for consumers. "I want to end this flagrant scam and put extra money straight into the healthcare savings account in your name," Trump said, arguing that consumer choice--not government mandates--should drive healthcare decisions.
The proposal also takes direct aim at pharmacy benefit managers and large brokerage middlemen, pledging to end kickbacks that Trump says artificially inflate insurance costs.

Eh. I dislike the price discrimination, I like the potential of paying less, but I'm not in love with the government intervention into the economy.

I'm also not animated about it. Big Pharma has revealed itself as mercenary and anti-freedom.

I'm not proposing that they be run as "non-profits" because we all know that scam, but I'm very uncomfortable with them being publicly traded. Because when you're publicly traded your very first priority is your shareholders (as it should be). In a word yes. But the Hasidic Community votes for Democrats so they look the other way. There are numerous counties just north of NYC where the fraud is rampant. 50 1) Big pharma spends billions of $ to develop a new drug. They patent it. They only get 14 years or so to recover the money invested. Hence, they need to sell the drug as fast as they can to everyone they can, worldwide, before it can go generic.

2) Other countries say, nice drug you have, but we'll only pay $10 a dose. Big Pharma needs $50 a dose to recoup and make a profit in 14 years. But $10 is > $0. So big pharma agrees.

3) Easy answer - they charge stupid Americans $300 a dose to make up the difference.

This has been going on for decades now. And more and more countries have gotten in on the scam - demand lower prices, big Pharma makes up the difference in the American market. And nobody says boo about it.

It's a big chunk of why drug prices have spiraled out of control in the U.S. - ALL the cost to create the drug is shifted to the U.S. Same is true for medical devices. The whole world forces you and I to pay for all medical advancements. Sad part is if every country paid a little more, we'd pay a ton less. We are 4% of population but pay 95% of the drug development costs.

The problem is that their only competitive pressure happens at the consumer level -- the public trying to save money by getting plans with less coverage. But Obamacare essentially outlawed that.

Or by people shopping around to reduce costs by using lower cost procedures or providers. This too has been made impossible.

Ideally, consumer would chose plans based on how much coverage they want and then still be able to reduce cost by not always defaulting to the most expensive drugs, most expensive tests, most expensive procedures.

I was in hospital in Japan and the Dr. actually asked me if I wanted an MRI. Said it wasn't really necessary, but for $70 we could check if anything else was wrong. (This was just at start of wuhan flu and hospital was mostly empty.)

So US says we need to allow imports to force uniform pricing.

So then FDA say, "Oh, BTW, it is OK to import shit made in China and India where we have no effective oversight (but we do have offices for when someone wants a vaycay) and they usually come with major problems."

You see, all decisions by gov. is what is best for industry. It is all about kickbacks and setting up a retirement fund for gov. workers.

144 My big issue with Big Pharma is that they are set up as an industry more focused on medical maintenance than curing disease/medical conditions.

The money is in ongoing customers, not outright conclusion. It pays better dividends to make an individual dependent upon medication to manage a medical condition, as opposed to outright curing. And, I'm not stupid. I understand for them to exist they have to cover the massive costs of their research and development (which are obscene, btw).

I think we should stop providing federal grants to pharmaceutical companies (who are the largest recipients) and focus on providing grants to medical research labs that are motivated by one thing, and one thing only....finding a cure to a particular disease.

The motivation for a Pharmaceutical company is financial. On a personal employee level it is stability. Finding a cure is about 7 or 8 down on the list.

You still can’t reduce the cost by more than 100% for the consumer.

If that aspirin costs $10 and you reduce the cost to $0 it’s a 100% reduction. Unless you start paying people to take the aspirin it’s mathematically impossible to see savings of 500% like the claim made.

Wrose - this has been blatant and obvious for decades. And the Euros demanded to be charged less, say they get a drug for $50, that we pay $100 for. Then they say, why stop at $50? $20. And we pay $150. And so on. Canada and Japan, hell, the whole world has been doing this.

It's how they can offer socialized medicine - by transferring as much of the cost as possible over to us. Drug companies, as much as I dislike them, are in a pinch - $20 is better than nothing. As long as they can make it up charging us, it all works out for them. So, they agree to the new prices. It's why drugs are radically cheaper in Mexico and Canada - they are selling the same damn drugs for less because they told the drug companies, they want the drugs for less. because the whole world is in on the scam, they might pay slightly more, but we'll pay a LOT less.
