Your link on the fatal DC mid-air, search NTSBgov for the briefings. Their channel is on YouTube and I think X. Basically the last minutes were as follows from the third NTSB briefing. Bluestreak was stabilized on final, descending. Below 500ft TCAS gave an audible "Traffic traffic" is heard on the CVR. FDR showed the plane's pitch was changing. Then on the CVR you hear the collision. Blanco Lirio discussed this briefing last night. A big takeaway for me was when PAT25 "HE" called out the first time he saw Bluestreak to ATC, PAT25 was about six miles away and would have been looking across Reagan to pick out the landing lights. Why was that helicopter flying into the flight path of landing planes at Regan. Its a well know flight path every plane follows if landing at the delegated runway. They were flying VFR into a very busy flight path with planes all following the same path to the runway.(s) As a retired pilot I was always warned about staying away from a busy runway unless lined up to land. The airliner was probably using the shorter runway because its a CJ and can use that runway. Radar follows all airplanes and I believe they were warned but they stayed VFR.

The real answer is there had never been a problem before. It took an undermanned ATC tower, a sudden switch of runways, a bad altimeter in the helo, and lack of situational awareness in the helo pilot in command. In aviation, safety advances one tragedy at a time. It is much safer now than in the past. And the situation around Reagan National will get cleaned up. Helps take away the human element of your soundings. Of course ATF should also do that, but they can't control what the pilot does.
