Pentagon Ponders Ketogenic Diet to Make Soldiers Stronger, More Lethal

Oh boy. Sorry about this. It just worked out this way.

I've had this post in the queue since this morning.

And having talked up keto all day in the comments, now I spring this on you.

But I swear, I just forgot I'd written this up earlier.

I'm not trying to do keto NXIVM, swearsies.

Carbs are the enemy, and the US military must terminate them with extreme prejudice.

Ditching carbs may be the key to military success in America's future wars.

Top Pentagon officials say research has shown that human bodies in ketosis -- the goal of the popular and controversial ketogenic diet -- can stay underwater for longer periods, making the fat- and protein-heavy eating plan a potential benefit to military divers. It is one example of a rapidly growing trend as military researchers zero in on how nutrition and certain drugs can enhance how fighting men and women perform in battle.

But U.S. defense officials say they lack the legal authorities to dictate to soldiers, sailors, airmen and Marines what they can and cannot eat. Critics of the entire concept warn that the military is entering a danger-filled world if it begins ordering diets and drug protocols solely to build more lethal warriors.

...

An Ohio State University study this year followed 29 people -- most members of the campus ROTC -- and had roughly half of them follow the keto diet. The study found that the 15 participants on the diet were able "to maintain ketosis for 12 weeks" by eating just 30 to 50 grams of carbohydrates each day.

Those participants lost an average of 17 pounds and 5% of their body fat, researchers say, while those in the group who followed a more carb-heavy diet experienced little change to their bodies.

...

"We showed that a group of people with military affiliation could accept a ketogenic diet and successfully lose weight, including visceral adipose tissue, a type of fat strongly associated with chronic disease," said Jeff Volek, a professor in the department of human science at Ohio State and a co-author of the report. "This could be the first step toward a bigger study looking at the potential benefits of ketogenic eating in the armed forces."

A doctor named Dominic D'Agostino is the main guy studying the effects of ketosis on divers, working with the Navy SEALs.

I think D'Agostino is mostly researching exogenous ketones -- ketones you eat as supplements, rather than endogenous ketones produced in the body via carbohydrate restriction -- but the end results are similar.

For the first time, ketone esters--oral supplements useful in epilepsy treatment--are being studied to fight seizures caused by hyperbaric oxygen toxicity, a life-threatening byproduct of breathing too much oxygen that impacts deep-water divers.

The Office of Naval Research (ONR) is sponsoring this research.

...

Dr. Dominic D’Agostino, a professor at the University of South Florida in Tampa, is conducting the ONR-supported research.

...

Special Operations divers such as Navy SEALs are especially at risk. Divers can encounter dangerous levels of nitrogen and carbon dioxide gasses when breathing underwater, requiring a rebreather to mitigate the toxicity. But Special Operations divers use a closed-circuit rebreather that filters out the gasses in such a way that bubbles don't appear on the water’s surface--useful when trying to avoid detection by enemy combatants.

However, this additional stealth increases how much oxygen the divers breathe and, combined with mission stress and physical exertion, can lead to seizures, convulsions, nausea, dizziness and even coma or death--all symptoms of oxygen toxicity.

Currently, anti-seizure sedatives are the only treatment for oxygen toxicity, said D'Agostino. But these drugs must be administered in high doses that could impair warfighters' mental and physical performance. D'Agostino sees a possible solution in ketone esters, which are supplements engineered to raise blood ketones--powerful energy sources the body produces naturally that act as a super fuel.

If you're interested in his work, there are dozens of YouTube interviews with him, including on Joe Rogan. Here's one of him discussing his research into using ketones/ketosis to reduce neurological impairments like epilepsy with Rhonda Patrick, who's a little nerd cutie.

In the first section of this interview, he talks about DARPA's interest in ketone supplementation. (The military doesn't like the full ketogenic diet, because they're worried about reduced performance without carbohydrates and they've got an entire sub-industry of food preparation already in place which includes a lot of carbs, so they'd prefer to just add ketones in pill form.)

He talks about his work with SEALs here as well.

Yeah, I mentioned that in an earlier comment thread. I can't say that it definitely affected my sleep, because I wasn't sleeping great before the fast, but I did tend to wake up more during sleep, and wake up earlier than I indended (often after just five or six hours).

Also, I tended to try to make up for the water I had not drunk during the day by drinking a lot at night, and I would have to get up and pee during the night at least once, sometimes twice.

I have read other people having this problem, sometimes because they feel too amped up due to the ketones.

I often didn't sleep well and I'm not sure if the fasting had something to do with this, but they did coincide.

Then again, I broke fast on Wednesday and still have not had great sleep, and still often have to pee in the middle of the night.

But other people report having sleep problems -- or, putting it in a better way, not needing as much sleep and waking up early and energetic -- so I think probably something went on with my sleep.

The bad days I had here and there were due to being tired from short sleep, not from weakness due to hunger.

35 People need to understand that for almost all of military history, the danger has been troops starving to death.

The only time weight gain was an issue was when they were plundering the hell out of something and getting drunk all the time.

So the military is going to have to make this change slowly. Fresh veggies take a lot more effort to ship out and store, while canned peaches in syrup last forever.

I've noticed they're starting to issue "diet" MREs that have a lot less calories in them.

I've heard that the military has been struggling with obesity in recent years. It's good to see that they're embracing state-of-the-art science.

83 Jane --

The Boy is Type 1, and I've never found any evidence to suggest that is reversible (by ketogenic diets).

Although long before it was cool, the Enigma (my oldest daughter) was on a medical ketogenic diet for five years and went from hundreds of seizures a day to none. You wanna talk overwhelming? Try measuring out GRAMS of carbs for your epileptic child: Get it wrong and watch her fall to the floor with grand mals.

Point is, it gets easier. And type 1 diet control is a bunch easier than a medical ketogenic diet. You'll have it down in no time.

That aside, The Boy can sometimes do low carb and ketosis, but you do have to be careful because you don't want to end up in ketoacidosis, and you no longer have a pancreas working to regulate insulin production.

93 But U.S. defense officials say they lack the legal authorities to dictate to soldiers, sailors, airmen and Marines what they can and cannot eat. Critics of the entire concept warn that the military is entering a danger-filled world if it begins ordering diets and drug protocols solely to build more lethal warriors.

I bet you put more thought into a single meal than I do into a month's worth of cooking. Without doing anything more complicated than skipping breakfast a few times and upping my workout routine I dropped a dozen pounds this spring. I decided that I'll push for a few more this week, but mostly all I'm doing is giving up snacks for a few days and making sure I get more exercise.

And I'm 29, so it's not like I'm a teenager and the pounds just fall off on their own.

Got my Captain to buy off on my plan. Changed it from working hours, to after working hours (as they were not meeting standards).

Then, they would do 100 pushups, 150 situps, and go 2.5 miles, after group calisthenics.

I did not care how long they took to do the 2.5 miles, they could walk it if they wanted, BUT they did not get to go home until they were done.

Actually had a much higher success rate then the program it replaced.

With Keto do you have to always be in a ketosis state?

...

It's not much different, though "Keto" is High Fat Low Carb (Moderate or Low Protein), whereas Atkins didn't really put any cap on protein, and actually tended to suggest meals with a lot of protein.

You don't always have to be in state of ketosis but that's when you're actually burning fat and lowering insulin level.

I overeat protein which actually does spike insulin (though not as much as carbs) so I often go out of ketosis after I eat. I'm having trouble with this "moderate to low protein" part of it.

Also, very few people will be in ketosis after any meal, for like an hour or whatever.

I mean, ideally you'd almost always be in ketosis, except post-meals, but it's not necessary. It's just good to be in it frequently. Most people who are not athletic or who don't work physically demanding jobs have probably not been in ketosis much outside of the last hour of sleep, since they were babies. (Babies are almost exclusively in ketosis for a while.)

A big issue is stress. GIs overseas can't drink or screw, so they eat.

Comfort food is all they have, so they dig into it.

And when they get home, they're less active. So it is an issue.

For the first time ever, we have a military that can eat itself into obesity, so adjustments need to be made.

Part of the problem is that we're dealing with volunteers. Conscripts you could just feed them tasteless gruel and they'd stay skinny, but volunteers expect decent grub or they won't re-enlist. So we give them the best food we can, and they eat too much of it.

It can, in a soft way. Most soldiers will eat in the mess and if the mess serves mostly low-carb stuff, that's what they'll eat. You don't have to put a gun to their head. Just serve ice cream with full fat heavy whipping cream and non-calorie sweetener. Suddenly their desert is keto, and no mandate was necessary.

yeah, but you also can't outrun a fork.

It's much, much easier to lose weight by controlling intake then it is by exercising.

158 It can, in a soft way.

Only if Joe eats in the chow hall. Often times, he doesn't. This is actually an issue: facilities are underutilized. You'd have to kick all of the restaurants off of post and prevent soldiers from leaving post during the duty day.

That's interesting. I got this keto tester from IndieGogo (KeyTo) a couple of weeks ago and noticed that when I broke a fast with a salad and a few croutons, I stayed at 5 (on a scale from one to seven) for a couple of hours, then bounced up to 4.

4 is supposed to be "light" ketosis, whereas 5 is more solid. (I have yet to get to 6 or 7. :-))

So far it seems pretty accurate. Takes me about 60 hours of fasting to get to 5, but once I'm there it seems to hang on pretty well.

This is breath testing, mind you. I don't know how that impacts things.

Unsympathetic managment defined. Bernard Montgomery took over command of 5th Corps after Dunkirk. In his words:

"I ordered that at every headquarters the whole staff, officers and men, would turn out on one afternoon each week and do a seven-mile run. This applied to everyone under forty, and there would be no exception; those who didn't want to run the whole course could walk and trot, but they must go round the whole course...

"I remember the case of a somewhat stout old colonel who went to the doctor and said that if he did the run it would kill him... I then said that if he was thinking of dying it would be better to do it now, as he could be replaced easily and smoothly; it is always a nuisance if officers die when the battle starts and things are inclined to be hectic. His state of health was clearly not very good, and I preferred him to do the run and die."

Coincidentally, people started getting super obese and getting diabetes like crazy after our school lunches, nutritional advice, serving advice on the back of packages, and everything else that follows government guidelines started being so carb heavy.

IIRC from school, we were taught to eat 8-10 servings of bread and pasta a day, 4-6 of vegetables, 4-6 of fruit, minimal meat, and at the verrry top was a small amount of sugar.

I will say that I've been eating a lot less and exercising a lot more for the last six months or so. No significant changes (down 1-2 lb), but it has been easier to maintain the same weight.

(I do have a theory, actually - I think my body is less efficient than others'. Like, for a given bolus of food, it extracts less nutrients and produces more waste (shit) than someone else's. This would explain why I don't gain weight and am always cold. I'm not sure whether I shit more than other people, though. Don't particularly want to test that.)

My husband loves that he can nuke it for a quick breakfast before hurrying out the door around 6:30 a.m. Total carbs are 6 gm, 16 gm protein, gluten free and grain free. One sandwich is 240 calories.

Two hours later when he tests his blood glucose, it's normal.

Quit for 7 months, then started juuling occasionally, and have been back to smoking a cig or two when drinking lately. Oh, and juuling a lot.

237 >>> Interesting. I eat terribly (loads of carbs and sugar. I made M&M cookies this weekend and have eaten 10 of them so far) and don't typically gain weight. I wonder why.

some people just don't have a problem with carbs. For other people, they have dysregulation of digestion of carbs.

Some of this is racial. American Indians, for example, are notoriously carb sensitive, which is why so many got obese after they were put on the government rations, high in carbohydrate.

There's an interesting story of the Pima indians, who were thin and muscular when they hunted and ate mostly meat. Then the government put them on a reservation and cut them off from any possible farming, and made up for that by giving the government rations, mostly rice and stuff.

They got hugely obese.

There are pictures of before and after of typical Pima indians in Taubes' book. (Also, it is generally acknowledged that Amerindians are just carb-sensitive.)

This is happening in a lot of places in the world where people are adapting what is called the Western Diet. Some are okay with it, but most become very unhealthy and obese.

Posted by: Harry Paratestes at June 11, 2019 08:14 PM (GBteo)

In my opinion, their whole definition of "serving" was totally fucked up and relates to nothing that normal people think of. Who the hell eats 8-10 "servings" of anything in the course of a day?

On my dinner plate, I have one serving of meat, one serving of potatoes, one serving of vegetables, etc. I think that's how people normally understand the term. Telling them to eat "8-10 servings" is demanding that they overeat.

It could also be the gluten.

I know people make fun of this, but my wife had a terrible time for years, gained weight, stomach pain, etc. and my eldest (who's a bit of a foodie) said "Try going gluten free."

So she did, the weight came off, she's healthier and more active, doesn't have to take three kinds of pills to fight various intestinal issues, etc.

Europeans have a high tolerance for wheat gluten. Amerindians do not. So they can't break it down and it makes them swell up.

As someone of Franco-German extraction, I can eat bread like a champ (mmm, pumpernickel with tuna salad...uhhhhhh), but it is carbs, so I limit it.

263 There's an interesting thing that happened starting the mid 1850s -- the poor used to have all of their teeth, and generally be thin and have low incidences of diabetes, etc., while the RICH tended to lose many teeth and be obese and have a lot of diabetes.

It's thought that's because only the rich could afford sugar (which used to be expensive) and fancy breads, etc. (Also, excessive calories.)

Anyway, once sugar and highly processed white bread (with barely any fiber) were cheap and affordable by the poor, the poor started losing teeth and getting obese.

Now the situation is reversed, as the rich tend follow better diets, and the poor eat what the rich used to.

297 >>>Certain carbohydrates play an important role depending on your type of activity and environment. You eliminate them in those situations, you die. Period.

yet I'm alive after 27 1/2 days of no calories at all (obviously no carbohydrates).

There is no such thing as an "essential carbohydrate." There are essential fats and essential amino acids -- stuff the body can't make itself and will die without - but the body can manufacture glucose (what carbs turn into in the blood stream before becoming more fat) as needed.

90% of the body's metabolism can run off ketones, which are made from dietary (ingested) fat as well as stored fat in the body. The 10% of required glucose can be made from protein (ingested, or from the body) as needed.

You're just wrong. The body doesn't "need" carbohydrates for energy. Both protein and fat are readily made by the body into energy.

Carbs can be useful, because they get into the blood fast, and so could be useful if taken an hour before a race or a powerlifting meet -- or just if you want extra energy for workout -- but they're not necessary to anyone's daily life.

304 You lucky fuckers with functioning immune systems can fuck around with these fad diets. Me? Notsomuch. My body not only doesn't produce insulin, but it doesn't make glucose either - which means I need a certain amount of carbs in order to function.

My lesbian sister in law however is on keto and having lost a shitload of weight - now has a coronary heart issue due to high cholesterol. I don't think she was doing keto the right way - when eating steak for example, she would eat mostly the fat/gristle and leave the actual protein alone.

Posted by: Tonypete at June 11, 2019 08:32 PM (Y4EXg)

Dude, that's the other thing -- once you get used to eating only one or two meals a day, it feels really weird and abnormal to wake up and eat food.

I went out to brunch with some friends and literally couldn't finish a small omelette.

I think we've trained ourselves to eat such a fucked up / abnormal diet in the western world, and it's only when you step away from it for a while that you realize how unhealthy it is.

You eat a carb heavy breakfast (cereal, or toast, or whatever), and it makes total sense that you're tired and starving by noon, where you eat a burger with french fries, crash at like 2pm, make it home and eat spaghetti, then go to sleep at 9pm.

Your insulin levels are so fucked all day ... of course you feel tired and lethargic all the time.

No, it becomes fat because they can't break it down. I'm told that flour tortillas are (for cultural reasons) considered superior to corn ones.

Amerindians can break down corn though, but not flour. So they pile on weight because it's not something they possess the enzymes to properly digest.

Add in all the sugars, and you get an even worse problem.

But the wheat gluten (which was added to almost everything to "fortify" it) is also a problem. Even soups have it added to thicken the broth.

334 >>>98 I think a lot of people who think they are gluten intolerant are really just healthier with fewer carbs.

Most gluten comes with a lot of carbs. Cut gluten and you are halfway there.

...

I don't know about that, but gluten sensitivity and the full version, Celliac disease, are definitely real. They understand the biological pathway to the problem. (The gluten protein (it is a protein) can slip through the lining of the small intestine in many people and then get into tissues where undigested food is not supposed to be -- where it is recognized, correctly, a foreign intruder and attacked by antibodies producing inflammation and many symptoms of being sick.)

As a friend with gluten sensitivity, "Sure, I can have a slice of pizza, if I want to sit on the toilet with the shits all night."

Obviously cutting wheat or carbs would stop the problem, but in my friend's case, he's always been lean/thin and really doesn't need to cut out all carbs.

Gluten-free is enough for him.

I don't know if it's in our constitution, but in NH, we have a law or something that says we have to be the first primary state.

370 also, Jason Fung and his assistant Megan Ramos do a lot of chats and have a lot of content on Intensive Dietary Management. Jason Fung is the doctor specializing in type 2 diabetes who reverses a lot of case of Type 2 Diabetes with a combination of keto and some fasting.

Fasting isn't really necessary unless you're specifically trying to reverse type 2 diabetes, though it can help if you think you have metabolic syndrome, which is a sort of pre-diabetes that many, many people have. I probably had it myself, when my resting insulin was pretty high. Now it's normal/lowish.

But anyway, Fung has three books out, the lastest one on fasting, diet, and longevity. "The Obesity Code" is a very newbie-friendly book that explains what all this talk of insulin resistance and metabolic syndrome is.

The Keto Connect people have a cookbook and also hundreds of recipes on their site, both in text and in YouTube videos.

From a blood sugar perspective no. I can drink a can of diet coke and my blood sugar will do nothing. I drink a can of coke and I'm hitting up my insulin pump's bolus button pretty dang quick.

379 Worst things about it is that it is 20x sweeter than sugar. So your candida party(!) and thrive. But the worst thing about it is that your body regulates when you think you've had enough sweet, and HFCS is immune to that regulator.

397 Ace, don't do zucchini fries. That's a recipe for a soggy mess. The general consensus seems to be that daikon radish works best as a potato stand in when you need something solid or don't like cauliflower. Jicama will hold up when deep fried, but that is a root that simply will not soften no matter how long you fry or boil it. I've seen recipes for turnip and rutabaga fries, too, but like I said, most folks tend to settle on daikon radish.

Personally, much as I ate the hell out of fries in my high carb days, I really don't miss them. I just eat a lot of bacon. It seems to solve a lot of food nostalgia problems.

404 A lot of these discussions come down to quality vs. quantity of life. If somebody knew when they were going to meet the great breadmaker in the sky then all the choices would be different.

405 oh, the low carb bread company at netrition is "The Great Low-Carb Bread Company."

It's... eh, kind of bland, doesn't really give you the feels that bread does.

There are a lot of simple bread hacks online. Most involve almost flour, then melting mozzarella cheese and cream cheese to provide the bulk of the 'bread" (both are pretty tasteless and disappear as far as taste), an egg, and some baking powder to give it a bit of a lift.

I haven't done that but John Ekdhal has and he says they work. He complains about the time it takes to melt the mozzarella and cream cheese on the stove. I said, 'They all say to microwave it for 30 seconds." But for some reason he avoids the microwave.

I've made about 15 kinds of bread substitutes ( I really hate coconut), and have decided I'm better off just having the bread once in a while.

The rye-pumpernickel swirl bread at the Kroger bakery claims to be 15 or 16 grams of carbs per 2 oz serving, as opposed to the 20-30 for regular bread. So, I have summa that sometimes, toasted and soaked in butter. Or, with butter and cream cheese. That's pretty good, too.

They had to boil the wort to make beer, that killed off the bugs. Wine making doesn't really involve water per se other than the water from the grapes themselves. I think fermentation in an of itself does not necessarily kill off bacteria. In fact, lots of stuff can go wrong with wine fermentation and then you end up with vinegar. It won't kill ya, but it aint wine.

447 **Okay, but how are you getting enough fiber to have a decent bowel movement? Every time I've done low carb, I've gotten constipated.**

So, from my experience, fiber doesn't really play into it. I know that's contrary to a lot of standard advice. But I eat mostly meat and fat and some minor bits of vegetables, and do not have any issues. In fact, I went full on carnivore, so only animal products, for a solid month about two years ago and had zero problems.

I suspect that when people have constipation on keto, it's most frequently due to an electrolyte imbalance. Taking a good quality elemental magnesium and making sure to salt their food properly tends to clear it up for most folks I've encountered.

Yes, but your body can make glucose out of protein and fat, via gluconeogenesis. After using stored sugar as glycogen. The Inuit, who eat zero carbs have a blood glucose of 80.

So it's vital to the cell but not a necessary dietary macro because the body will make it.

I don't reject it. I think it is pretty obvious that it works. I also believe that certain people can't lose weight eating a lot of carbs. But I am also skeptical about new discoveries and fads. These health writers contradict themselves all the time. First they say don't drink sugary soda. Then they say drinking diet soda is just as bad. Some even say carbonation will get ya even if it is just water. ZZZZThere is no doubt something in the Western Diet of the USA/UK that has spread around the world is making people fat. But people from China, Japan, India, and even Italy certainly have been eating a lot of carbs for centuries and they were thin at least until recent decades. I don't buy the paleo argument that man was not mean to eat carbs like rice. It is too simple and ignores too many who have been just fine eating those things. Maybe it is just processed foods.

481 Oooh, diet thread! I have questions!

I do hope some Hordesters might have insight or experience to share, please.

I am interested in the supposed benefits of the carnivore diet, but I am concerned about reports of food reactions after going meat-only for awhile and then re-introducing other foods. I am suspecting part of the problem is just that the body stops producing digestive enzymes for the restricted foods and that is what causes the problems if there is no actual food intolerance. Kind of like, I could drink milk with no problems until I went for awhile without drinking it and then after that I had distress if I drank it. If I hadn't stopped drinking it, I think I wouldn't have developed the intolerance.

So, I am leery about eating only animal products for 30-90 days because I may lose tolerance for a lot of foods I want to be able to eat now and then.

But some say the healing only happens if one cuts out all plant foods.

Can't I do this with, say, 80% animal and 20% plant and still enjoy benefits?

Like I said, I don't have the answer. I can look in a mirror and know that lol. But people in Europe have eaten bread for centuries as a staple and people in the East have eaten rice. The other things they ate were normally unprocessed natural foods like fish, way more greens than we eat, and meat a times. Then there is the lifestyle issue, I can't see how that can be overlooked. Most people today do a lot more sittin' around.

Thanks, and you too. I rejoice in your improved health. I really do.

I'm questioning the causal relationship that is being asserted, that's all. Recall that until relatively recently all physicians considered ulcers to result from stress.

Why did they think that? Had they performed any research on this? No. Because they were sitting in lecture theaters listening to people like me telling them that. They had no more intellectual engagement with this issue than a bullhorn has with the speaker at a leftist rally.

Physicians, including those from Notre Dame - BS, Case Western - MD, dutifully parroted this. Then two Australians posited ulcers as resulting from a bacterial infection, and were laughed at by physicians from Notre Dame - BS, Case Western - MD, inter alia.

Turns out that they were right, flew to Stockholm for their Nobel Prizes, and all of the people teaching soon to be physicians were wrong.

The question here is epistemological, viz., how does one know what one thinks one knows?

I'm not. I'm defending reason.

And I detest the expression "good for you," except in the context of responding to someone doing something.

Let me ask this question: how does one know - empirically, not anecdotally - whether some food is "good for you" or not?

The problem with fast food is its high caloric content, which makes it easy to ingest way more calories than one can burn off. Think about this: who routinely eats fast food? People who are too lazy to cook, right? Now ... are such people more likely to be fitness fanatics, or to go back home and watch TV?

OK smartass, trans-fats are not good for you. But go right on defending junk food.

As far as your thermodynamic argument, I assume you were going for the argument that fat has 9 cal/g so therefore it burns slower than carbs.

That's not how it works and that's old tried BS. Not all macros are equal, and fat is more satiating so you don't eat as much.

But yeah I'll just go ahead and listen to an Internet troll instead of a Stanford professor of endocrinology who is challenging the status quo with data and results.

Pee sticks say I am constantly in ketosis (although not always deeply), diabetic blood sugars are great, nearly always

Also did some 14 hour fasts a couple of days a week in the last few weeks.

Not lost a pound. weight stays at the same point give or take 4 pounds which is equivalent of water weight.

I could be putting it on in muscle, I believe I have gotten a bit leaner, but I can't put muscle on as fast as I can loose the same weight in fat.

Agree though, carbs are the debbil, especially as a diabetic.
